Resident Care Coordinator/CNA

Job Description Support HWD by managing Assisted Living team members. Schedule Assisted Living team members. Participate in Care Plan meetings with families and residents and manage ISP’s as resident needs change. Promote all company policies by ensuring SLC communities are providing the best care possible for our residents. Participate in weekly at risk meetings with HWD, ED, MCC/ MCD to ensure leadership team is aware of all resident needs. Review and implement all necessary processes for optimal operational efficiency such as- shower schedules, laundry schedules, caregiver assignment sheets, etc. Qualifications Level of Formal Education: HS or GED Area of Study: General Years of Experience: 3-5+ years caregiver experience preferred Type of Experience: CNA experience preferred, scheduling, communicating with families Special Certifications: CNA preferred Language Skills: Acceptable fluency in English in order to perform job duties and speak, read, write, and communicate with supervisors, coworkers, residents, and applicable third parties.
