Supervisor of Clinical Services Reports To: Director of Clinical Services / Regional Clinical Director Department: Clinical Services Location: Connecticut (Fieldbased / Homecare and facility visits) Job Type: FullTime Position Summary The Supervisor of Clinical Services will oversee and coordinate the daily clinical operations and care delivery teams for Affinity Hospice of Connecticut. This role ensures that our hospice patients and their families receive compassionate, highquality care in accordance with our mission and regulatory standards. The Supervisor supports clinical staff (RNs, LPNs/LVNs, hospice aides, social workers, bereavement staff) in implementing interdisciplinary care plans, managing staffing and scheduling, monitoring quality and compliance, and fostering a culture of teamwork and continuous improvement. Key Responsibilities Supervise, mentor, and support clinical staff across home care, assisted living, and skilled nursing facility settings. Ensure timely assessments, plan of care development, and interdisciplinary team (IDT) meetings for hospice patients. Monitor and ensure compliance with state and federal regulations (including Medicare hospice benefit requirements, documentation, licensure) as well as internal policies and accreditation standards. Oversee staff scheduling, oncall rotation, visit coverage, and staffing models to maintain patient care continuity.
