Responsibilities

As a Home Health Licensed Practical Nurse, You will: 

Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
Valid driver’s license, acceptable driving record and proof of car insurance
CPR professional certification
1+ years of nursing experience preferred
Home Health care experience preferred

Position Summary: Provides and documents individualized patient care to include patient evaluation, education, medication administration, IV therapy, treatments and patient safety as directed by the RN or provider. Shares own knowledge with colleagues. • Provides anesthesia equipment and supplies based on identified needs in a timely, efficient, and safe manner. • Assists anesthesia personnel with the administration of general, spinal, and regional anesthesia. • Sets up all invasive monitoring lines as requested by anesthesia personnel. Under the direction of the Anesthesiologist/CRNA/SRNA, runs point of care labs and diagnostics, completing all required Point Of Care (P.O.C.) documentation. • Works closely with anesthesia staff during anesthesia emergencies and trauma cases; assisting with preparation of the O.R. suite, line placement, handling of blood and lab draws and patient positioning for procedure. • Troubleshoots and cares for all equipment used in the delivery of anesthesia care. • Receives, sorts, decontaminates, and cleans reusable anesthesia equipment. • Maintains and insures adequate inventory levels of anesthesia supplies in departmental service areas. • Assists with positioning the patient according to operating room policy and procedure, and transport post-operatively to the PACU or unit in a safe and caring manner for the patient. • Communicates appropriate information regarding the patient condition to the Anesthesiologist/CRNA/SRNA and other members of healthcare team. • Contributes to the department’s effectiveness by reporting to duty on time, adhering to policy concerning break and lunch, and completing assigned duties in a efficient, effective, and timely manner.
