On an Average Day You Will: (includes but not limited to) Perform personal care needs after given a patient's assignment and attend to his/her requests promptly; if unable to perform a certain task, report to the Case Manager immediately Provide patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality Meet safety needs of patient and use equipment safely and properly (foot stools, side rails, O2 etc.) Provide personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned Assist in dressing and undressing patients, as assigned Plan and prepare nutritious meals, including shopping, as assigned Assist in feeding the patient, as assigned Take and record oral, rectal and auxiliary temperatures, pulse, respiration and blood pressure when ordered with appropriate completed/demonstrated skills competency Provide proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence Assess and report on patient's condition and significant changes to the Case Manager; also, being aware of the caregiver or other individuals living with the patient and interpersonal issues Assist in ambulation and exercise as instructed by the hospice nurse or therapist Provide normal range of motion and transfers/positioning, as directed Assist with self-administered medications as allowed by state regulations, such as “reminders” Offer and assist with bedpan and urinal aid, as assigned Aid with light laundry needs if necessary Perform range of motion and other simple procedures as an extensional therapy service, as ordered with appropriate completed/demonstrated skills competency Provide respite for patient's family/caregiver when on-site, as appropriate Maintain patient's living area is clean and orderly, as assigned Adhere to the organization's documentation and care procedures and standards of personal and professional conduct Participate in quality assessment performance improvement teams and activities Respect patient and family/caregiver environment and patient's personal needs All other duties as assigned Requirements: Must be a Certified Nursing Assistant ( applicable for all States, additional requirements for California residents, see below). CALIFORNIA REGULATION : Must be a CNA in good standing AND must have a current Home Health Aide (HHA) certification in good standing in the State of California. Description At Cornerstone Specialty Hospitals , patients receive acute care from a physician-led care team specializing in treating patients with complex conditions. Our attending physicians see their patients every day, and physicians with specialties in pulmonology, cardiology, infectious disease, and general surgery may also participate in your care. Our nurses are also experienced in treating complex conditions, and all nurses in high-acuity units are certified in Advanced Cardiac Life Support. Additionally, patients with serious respiratory illnesses have 24/7 access to a respiratory therapist. Job Summary Assists professional nursing personnel in providing patient care in assigned area. Assists patients with activities of daily living, provides for personal care, emotional support and performs more complex clinical skills under the direction of professional nursing personnel. Essential Functions Consults with and keeps supervisor informed of unit activities, needs, and problems related to patient care. Provides general nursing care such as positioning the patient, lifting, turning, applying/utilizing special equipment, assisting in the use of bed pan, urinal or commode, ambulating the patient, pre operative and post operative care, and application of binder or anti embolic stockings, bed bath, oral/hair/back care, changing bed linens, cleaning over bed table, and bedside stand, straightening patient room and other general care necessary during the shift. Takes and records temperature, pulse, respiration, weight, height, blood pressure, and intake and output measurements accurately. Documents in a timely manner. Prepares patient room and bed for admission and transfers and ensures all necessary equipment is in room. Maintains an attractive and comfortable environment for patients with special consideration to cleanliness of room, ventilation, and lighting. Assists in patient admission, transfer, and discharge procedures. Assist with handling and care of patient belongings and other personal property. Answers patient call lights, telephone, and paging system and responds as appropriate. Under the direction of the Registered Nurse, performs routine treatment procedures such as enemas, application of appliances for heat and cold. Able to release, remove, and reapply restraints to patient under the direction of an RN. Ensures patient is in proper position and alignment. Assist Registered Nurse and/or LVN/LPN with complicated patient treatment procedures. Serves and removes patient meal tray in timely manner. Assists with feeding and/or preparing items such as opening milk container, cutting food for patient. Knowledge/Skills/Abilities/Expectations Must read, write and speak fluent English.
