The primary purpose of the Licensed Practical Nurse position is to, in accordance with professional nursing standards, skillfully administer total patient care based on the individualized needs of assigned patients and families. Under the direction and supervision of a Registered Nurse, provides technical nursing care to assigned patients, performs treatments and procedures within established guidelines, and provides a safe and therapeutic environment.

Among the key duties of this position are the following:
  • Participates in data collection related to the patients’ status and needs. Provides input to Registered Nurse for incorporation into the plan of care by communicating information pertinent to the patient’s status.
  • Provides direct patient care within scope of practice to ensure implementation of the plan of care.
  • Informs and instructs patients/significant others on patient’s health/illness state and plan of care.
  • Assists patient/significant other in preparation for discharge and health maintenance in the home.
  • Documents all aspects of patient care

Job Description The Ambulatory Licensed Practical Nurse is a member of the collaborative interdisciplinary team. The practice of nursing as a Ambulatory Licensed Practical Nurse is defined as performing tasks and responsibilities within the framework of case finding; reinforcing the patient and family teaching program through health teaching, health counseling and provision of supportive and restorative care, under the direction of a Registered Nurse or licensed Physician. The Registered Nurse is responsible to oversee any necessary coordination of the interdisciplinary plan of care and provides for its consistency with the medical treatment plan. Licensed care providers responsible for utilizing the care process relative to their scope of practice and knowledge base, document both objective and subjective physical findings and interventions. All patient care activities are administered according to standards, organizational policy and procedural guidelines. The Licensed Practical Nurse actively supports performance improvement initiatives by participating in educational activities. Internal Medicine 3-5 years clinical experience in medical/ surgical required; strong organizational skills preferred.
