Job Summary

GENERAL SUMMARY

We are seeking a highly skilled, compassionate Registered Nurse (RN) to deliver high-quality, patient-centered care through a combination of in-home patient visits, telehealth patient consults with Nurse Practitioners or Physicians as required, and administrative tasks.  The CareConnect Team at WellBe supports timely response to patients’ changing needs and this role will be instrumental in delivering this care consistently and in a patient-first manner.

This role primarily supports urgent/episodic interventions, working in partnership with physicians, nurse practitioners (APPs), and interdisciplinary care teams to address a range of patient needs across the care continuum. 

The RN will operate with a patient-first mindset, balancing strong clinical judgment with care coordination, education, and documentation responsibilities. The role has no direct reporting responsibility but will function as part of an integrated, interdisciplinary patient care team. Urgent Care RN staff in these roles will help foster positive and effective working relations with senior leadership and management, local market management and business ancillary departments for the purpose of achieving the company's vision, mission and objectives. S/he is responsible for following the rules and regulations of state and federal regulatory agencies and other certified agencies for their region.  Daily, local travel to meet patients and the community will be an essential part of your role.

Job Description

Salary: $37.25 - $54.12 per hour + Mileage & Benefits

Schedule: Saturday and Sunday, 8am - 5pm

  • Demonstrate and uphold the WellBe mission by helping patients lead healthier, more meaningful lives through the delivery of CompleteCare™.
  • Deliver urgent, in-home, patient care under the direction of a licensed provider throughout the assigned market including initial assessments, routine follow-up visits, and ongoing care coordination.
  • Conduct comprehensive home safety evaluations and document findings during patient visits.
  • Deliver patient care under the direction of a licensed provider by:
  • Obtaining patient histories
  • Reviewing medications and treatment plans
  • Performing physical assessments
  • Identifying and addressing health concerns observed or reported during home visits
  • Perform clinical interventions as appropriate and at the direction of the overseeing provider, including medication administration (PO, SL, IM, IV), wound care, point-of-care testing, phlebotomy, Foley catheter care, IV fluid administration, and other approved nursing procedures.
  • Maintain accurate, timely, and complete patient records, ensuring documentation is updated at each patient encounter.
  • Provide patient and caregiver education as directed by the APP-Nurse Practitioner and interdisciplinary care team.
  • Deliver high-quality, patient-centered care while meeting established clinical, quality, and operational performance standards.
  • Support care transitions, end-of-life discussions, palliative care interventions, and the closure of preventive care and HEDIS quality gaps, including screenings and medication adherence initiatives.
  • Complete and submit all required documentation, reports, and administrative paperwork within established timelines.
  • Participate in Interdisciplinary Team (IDT) meetings, providing updates and insights from patient and family interactions to support care planning.
  • Collaborate closely with APPs, nurses, social workers, behavioral health specialists, and other care team members to develop and implement individualized patient care plans.
  • Attend and actively participate in required training programs, including medication administration, clinical equipment use, new protocols, and ongoing professional development opportunities.
  • Build and maintain positive relationships with local healthcare providers, community organizations, caregivers, and referral sources to support WellBe's presence and reputation within the community.
  • Utilize available internal and external resources to address patient needs, including behavioral health services, community-based programs, and other supportive care resources.
  • Maintain all required active, unrestricted state licensure and certifications.
  • Adhere to professional standards of practice, ethical conduct, patient confidentiality requirements, and organizational policies.
  • Perform additional duties and responsibilities as assigned to support organizational goals and patient care excellence.
  • Scheduled working hours: 10am-7pm EST (tentative)

What you bring

 

Educational/Experience Requirements:

  • Active State Licensure required; compact licensure preferred.
  • 3+ years as an RN in a direct, patient-facing care delivery role (required).
  • CPR Certification is required.
  • Prior Telehealth experience (preferred).
  • Home care experience (preferred).
  • Previous experience in hospice, palliative care, or geriatrics strongly preferred; other complex patient health care experience will be considered.
  • Possess a valid driver's license with an acceptable driving record.
  • Requires proof of an active auto insurance policy.

 

Required Skills and Abilities:

  • Ability to conduct in-home setting visits and have previous experience on a telehealth platform
  • Demonstrated role proficiency with Microsoft Office; Excel, Word, G-suite, etc.
  • Proficient with Microsoft 365 software suite, Electronic Health Records, Scheduling Software, and Ring Central applications
  • Role model in courageous teamwork, integrity, ethics, and productivity
  • Excellent verbal, written, presentation, and interpersonal communication skills
  • Strong communication skills, with the ability to effectively interact with patients, families, and remote healthcare professionals.
  • Ability to work independently and make sound decisions while adhering to established protocols and guidelines.
  • Excellent organizational and time management skills to handle administrative tasks and facilitate telehealth consultations.
  • Experience working with culturally diverse and underserved populations.

 

Supervisory Responsibility: This position will not have supervisory responsibility.

 

Travel requirements: Travel is required up to 100% locally to deliver patient care.

 

Work Conditions: Ability to lift up to 20lbs.  Moving lifting or transferring of patients may involve lifting of up to 50lbs as well as assist with weights of more than 100lbs.  Ability to stand for extended periods. Ability to drive to patient locations (ie. home, hospital, SNF, etc) or community resource locations. Fine motor skills, and Visual acuity.
