Licensed Vocational Nurse (LVN)

Golden Haven Care Center is seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our 99-bed Skilled Nursing Facility in Glendale, CA. Under the supervision of a Registered Nurse (RN), the LVN is responsible for providing quality patient care, applying the nursing process, and supporting the overall well-being of our residents.

Position Summary

The Licensed Vocational Nurse (LVN) is responsible for delivering high-quality nursing care in accordance with established standards and facility policies. The LVN works collaboratively with the interdisciplinary team to assess, implement, and evaluate patient care while promoting a safe and compassionate environment for residents.

Schedule

Full-Time
Rotating 4 days on / 2 days off schedule.
Work performed at a single location in Glendale, CA.

Available Shift

PM Shift: 3:00 PM – 11:00 PM

Pay: $38.00–$43.00 per hour, based on experience.

Responsibilities

Utilize the nursing process to assess, plan, implement, and evaluate patient care.
Conduct focused patient assessments to identify individual care needs.
Monitor and assess residents for physiological and psychosocial changes, reporting significant findings promptly.
Collect, analyze, and document patient information and nursing observations accurately.
Administer medications safely and in accordance with physician orders and facility policies.
Maintain complete and accurate medical documentation.
Collaborate with physicians, Registered Nurses, and other healthcare professionals to ensure quality patient outcomes.
Perform additional duties as assigned.

Qualifications

Current California Licensed Vocational Nurse (LVN) license (required).
Current BLS Certification (required).
Minimum 1 year of LVN experience in a skilled nursing or related setting preferred.
Ability to communicate effectively in English.
Authorized to work in the United States.
