Job Summary:

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Auburn Skilled Nursing and Rehabilitation in Salem, Ohio. As a valued member of our care team, the LPN will provide high-quality nursing care to our residents and contribute to our mission of providing individualized, compassionate, and innovative healthcare services.

Responsibilities:

  • Provide direct patient care to residents in accordance with established policies and procedures

  • Administer medications, treatments, and interventions as prescribed by physicians

  • Monitor and report resident condition, including vital signs and any changes or concerns

  • Develop and implement individualized care plans in collaboration with the interdisciplinary team

  • Communicate effectively with residents, families, and other healthcare professionals

  • Maintain accurate and detailed records of resident care and treatment

  • Participate in quality improvement initiatives and strive for excellence in patient care

  • Collaborate with other nursing staff to ensure seamless care transitions and resident needs are met
