We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team at Carease Health - PA, a nationwide preventative healthcare company focused on reducing hospitalizations. As a Nurse Practitioner, you will deliver high-quality preventative care to residents in skilled nursing facilities, combining on-site visits with remote work from home.

Responsibilities:

  • Provide preventive care to residents in skilled nursing facilities, with a focus on Geriatric care and Internal Medicine

  • Conduct on-site visits and remote work from home, supported by a dedicated scribe team for efficient documentation

  • Build strong relationships with facility staff and maintain a patient-centered approach

Requirements:
