Job Summary:

We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team at Carease Health - PA, a nationwide preventative healthcare company focused on reducing hospitalizations. As a Nurse Practitioner, you will deliver high-quality preventative care to residents in skilled nursing facilities, combining on-site visits with remote work from home.

Responsibilities:

Provide preventive care to residents in skilled nursing facilities, with a focus on Geriatric care and Internal Medicine
Conduct on-site visits and remote work from home, supported by a dedicated scribe team for efficient documentation
Build strong relationships with facility staff and maintain a patient-centered approach
Collaborate with healthcare teams to develop and implement personalized care plans
Conduct thorough assessments, diagnose and treat patients, and develop treatment plans
Maintain accurate and timely records, adhering to facility and regulatory requirements
Participate in education and training programs to stay current with best practices and emerging trends

Requirements:

Active NP or PA license (in the state of practice)
1+ year of experience in Geriatric care and documentation review
