Summary
Coordinates nursing care for Adult/Geriatric residents on the LTC unit. Conducts, directs, and participates in resident, family, and staff teaching and provides leadership by working cooperatively with ancillary nursing and other resident personnel in maintaining standards for professional nursing practice. Actively contributes to care planning and managing resident care via participation in the Interdisciplinary Team (IDT) and other committees as requested, such as Behavior Management Committee. The Quality Assurance RN assists in resident care, including but not limited to wound care, infection control, or assuming a cart as needed. This position assists the Director of Nursing with Infection Prevention monitoring and oversight, as requested. The Registered Nurse is seen as a role model for exhibiting the Behavioral Standards and Customer Service expectations of MyMichigan Health.
Responsibilities
(25%)* Standard of Care. Maintains the standards of nursing care and implements the policies and procedures of the hospital and nursing department, which includes maintaining the Resident’s Bill of Rights.
(25%)* Resident Care – Assessment & Planning. Performs thorough, timely and accurate assessments of residents. Appropriately utilizes equipment necessary in the assessment process. Documents and communicates assessment results as required. Reassess residents as needed and per protocol. Delegates according to the state scopes and standards of practice appropriate data collection to LPNs and Nurse Aides. Participates in the development of individualized nursing care plans. Prescribes appropriate nursing actions towards meeting the goals established in the care plan; including, but not limited to, physical, psychological, social, rehabilitative, educational, and discharge planning needs of the patient. Evaluates and modifies the nursing plan as needed. Documents the plan and the resident’s progress. Participates in resident care conferences.
(25%)* Resident Care – Implementation & Education. Uses professional nursing skills and judgment to administer direct nursing care based on assessment, care plans, and physician orders; including, but not limited to, medication administration, performing prescribed treatments, utilization of medication equipment/supplies and assisting physicians in special testing and procedures. Documents and communicates intervention per policy. Institutes emergency measures for sudden adverse developments in residents. Determine and prioritize specific resident/family educational needs and provide education to meet the identified learning needs. Encourage and promote interactive communication and decision making about care with resident/family. Implement plan for education based on assessment of learning needs, abilities, preferences and readiness to learn. Consider barriers to learning. Collaborate with other providers to coordinate education. Documents educational activities. (25%)* Assignments. Assigns nursing care to team members in accordance with the resident’s needs and the personnel’s capabilities and qualifications and assigns other duties as needed. Directs, supervises, and evaluates nursing care provided to residents. Participates in orientation of new personnel. OTHER DUTIES AND RESPONSBILITIES: Develops and maintains good working rapport with interdisciplinary personnel to meet the needs of each resident. Demonstrates the ability to organize and prioritize the needs of groups of residents or an individual resident. Demonstrates a general ability to manage assignments and perform routine duties. Documents in an accurate, clear, and concise way and includes pertinent clinical data. Shares information and reports significant changes and/or abnormal findings to appropriate team members. Participates in regulatory surveys. Participates in the performance improvement activities. Other duties as assigned.

EXPERIENCE, TRAINING AND SKILLS: Geriatric Certification recommended/preferred. LTC specific Infection Prevention certification within 6 months of hire or transfer preferred.
