Job Summary:

Synergy HomeCare of the West Valley is seeking a compassionate and skilled Home Health Caregiver to join our team in Goodyear, Arizona. Responsibilities:

Provide direct patient care, including assistance with activities of daily living (ADLs) such as bathing, dressing, and grooming
Assist with medications administration, medical equipment, and medical treatments as needed
Monitor and report changes in patient condition to the healthcare team
Maintain a clean and safe environment for patients, including disposal of biohazardous waste and cleaning of equipment
Develop and implement personalized care plans in collaboration with patients, families, and healthcare teams
Document patient care and communication with healthcare teams in a timely and accurate manner
