Activity Aide

Summary/Objective

In keeping with our organization’s goal of improving the lives of the Guests we serve, the Certified Nursing Assistant (C.N.A.) plays a critical role in providing superior customer service and nursing care to all Guests. The C.N.A. safeguards the health, safety and welfare of all Guests under their care by following applicable laws, regulations, and established nursing policies and procedures.

Essential Functions

1. Provides quality nursing care to Guests in an environment that promotes their rights, dignity and freedom of choice. 2. Provides individualized attention, which encourages each Guest’s ability to maintain or attain the highest practical physical, mental and psychosocial well-being. 3. Carry out assignments required for the Guest’s activities of daily living (ADL’s) which include but not limited to bathing, dressing, grooming, toileting, and feeding. 4. Attends to individual needs of all Guests in regards to incontinent care, transferring, ambulation, range of motion, communication and other needs. 5. Provides care that maintains each Guest’s skin integrity to prevent pressure ulcers, skin tears and other damage by changing incontinent Guests, turning, repositioning immobile Guests and by applying moisturizers to fragile skin and other areas. 6. Performs various tasks assigned by the floor nurse including checking vital signs, weighing Guests, applying creams/ointments and collecting specimens. 7. Must be knowledgeable of individual care plans and support the care planning process by providing supervisors with specific information and observations of the Guest’s needs, preferences and report any behavioral changes. 8. Assists the Wound Care Nurse as directed. 9. Completes medical records documenting care provided and other information in accordance with nursing policies while maintaining strict confidentiality. 10. Must answer and respond to call lights promptly and courteously. 11. Assist with new admissions, re-admissions and assist with the transfer of Guests to different rooms within the facility. 12. Communicates and interacts effectively and tactfully with Guests, visitors, families, peers and supervisors. 13. Attend and participate in departmental meetings and in-services as directed. 14. Reports all resident concerns to the appropriate department head.

16. Recognize your role as part of the Quality Assurance and Performance Improvement 

(QAPI) efforts of your organization. 

17. Attend trainings to build understanding and capacity to undertake QAPI work. 

18. Carry out QAPI roles and responsibilities as assigned. 

19. Follow established policies and procedures in support of QAPI efforts. 

20. Any noted opportunities for improvement will be shared with leadership in the 

organization. 

21. Communicate to leadership upon witnessing a positive outcome of a QAPI project or 

detecting barriers preventing project success. 

22. Support QAPI efforts both verbally and non-verbally (i.e., via actions and attitude), 

including adjusting performance and practice in accordance with QAPI initiatives and 

findings. 

23. Participate in compliance trainings as scheduled and adhere to compliance plan. 

Work Environment

This job operates in a health care setting. This role requires regular walking to various locations 

around the facility while pulling or pushing carts. This position works mostly in the resident 

care areas as well as in occasional outside weather conditions. This role may also come into 

contact with patients who may have contagious illnesses. 

Physical Demands 

While performing the duties of this job, the employee is regularly required to talk and hear. This 

position is very active and requires standing, walking, bending, kneeling, and stooping on a 

regular basis. The employee must be able to lift and move items weighing at least 20 pounds. 

Specific vision abilities required by this job include close vision, distance vision, color vision, 

peripheral vision, depth perception and ability to adjust focus. 

Position Type and Expected Hours of Work

This position is part of a health care facility that is open 24 hours a day, 365 days a 

year. Rotating shifts as well as holiday and work during inclement weather will be required.

Required Education and Experience

• High school diploma or equivalent.

Preferred Education and Experience 

 One year experience in an Activity Aide role
