Position Summary
We are seeking a compassionate and experienced Registered Nurse (RN) to conduct in-home care assessments for individuals referred for home care services. This part-time position offers flexible scheduling and the opportunity to help clients receive the appropriate level of care based on their individual needs. Assessment visits typically last 30–60 minutes and focus on evaluating each client's health status, functional abilities, living environment, and support needs to develop appropriate care recommendations.
Responsibilities
Review referral information and supporting documentation prior to each scheduled assessment.
Contact the client, family member, caregiver, or designated representative to schedule the in-home intake.
Conduct comprehensive in-home assessments, including evaluation of:
Client’s medical, cognitive/emotional, and physical health status
Functional abilities and Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs)
Home safety and environmental risks
Caregiver support and additional service needs (which include the number of hours, time of day, and days of the week the client wants home care services)
Identify and recommend appropriate home care services based on assessment findings.
Communicate assessment findings, complete assessment documentation, recommendations, and any concerns promptly to the Care Coordinator.
Qualifications
Current, unrestricted Registered Nurse (RN) license in good standing.
Minimum of three years of geriatric nursing experience required.
Experience in home care, home health, case management, geriatrics, community health, or care coordination preferred.
Strong clinical assessment and critical thinking skills.
Excellent communication, documentation, and interpersonal skills.
Ability to work independently and effectively manage a flexible schedule.
Reliable transportation, a valid driver's license, and willingness to travel throughout the assigned service area.
Sales experience preferred.
Schedule & Compensation
Position paid hourly, $35-$40/hr depending on experience, with flexible schedule and variable hours week-to-week.
Assessment visits generally last 30–60 minutes plus prep work, travel time, and mileage all of which is paid
Travel to clients' homes within the assigned service area is required. Service areas may expand or change based on agency needs.

Communicate assessment findings promptly to the Care Coordinator to support timely care planning.
Represent the agency with professionalism, compassion, integrity, and respect during every interaction with clients, families, caregivers, and referral sources.
