Office Location: N. 168 West 22730 Prairie View Lane, Jackson, WI 53037

Coverage area: Travel throughout the territory is required- Primarily Washington, Ozaukeend northeast Milwaukee Counties

Schedule: Full Time Monday-Friday 8AM-5PM

Responsibilities

As a RN Case Manager You will:    

  • Assess/monitor physical, emotional, and psychological needs of patients 
  • Create hospice care plans that align with the patient's wishes and goals   
  • Direct nursing care: administering medications, treatments, and interventions   
  • Provide pain and symptom management  
  • Educate and support the patient’s family and caregivers 
  • Collaborate with interdisciplinary team 
  • Maintain accurate and timely documentation  
  • Participate in on-call rotation as required by local branch   

Qualifications

    • Graduate of an accredited nursing school with current licensure to practice in state of operation 
    • One year nursing experience required, 3+ years preferred 
    • Experience in hospice or a similar setting preferred 
    • Knowledge of the hospice philosophy of care 
    • Commitment to clinical and documentation excellence 
    • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order

Registered Nurse (RN) - Home Hospice and Palliative Case Manager -0.5 FTE- Days

What You Will Do: The RN Case Manager works collaboratively with patients, social workers, physicians, other practitioners, caregivers and community agencies in coordinating and overseeing the interdisciplinary plan of care for a patient in a home care/home hospice environment. Assesses, plans, facilitates and evaluates timely coordination of high quality, cost-effective care. Responsible for improving the health of patients dwelling in their own homes. This requires a nurse who is comfortable working independently, enjoys teaching, is a critical thinker and has strong communication skills. The RN Case Manager provides hands on care in the home environment and offers autonomy, flexibility and the luxury of caring for one patient at a time. Ideal candidates also possess negotiation skills along with strong analytical and computer skills. Experience with pre-acute and post-acute care as well as community resources is preferred. Case Managers also promote safety, cost efficiency and a commitment to quality improvement. What you will need: Associate’s Degree in Nursing required. Bachelor's Degree in Nursing preferred. Current State of WI or eNLC compact state RN license. Valid State of WI driver’s license with an acceptable driving record. Current BLS (CPR) certification or ability to obtain upon hire.
