Job Overview

The RN independently provides direct nursing care to patients and their families in accordance with established policies, procedures, and practices.

Location: Riley 3 (R3) Unit- SaintsCampus

Hours: 36 Hours (Full-Time) Weekend and Holiday Commitment Required

Shift: Evenings/Nights 7:00PM-7:00AM

The Community RN Career Direct Care Pathway placement is comprised of four levels based on a RN’s minimum experience, education, and qualifications.

Level I - 0 – Less than 12 Months of RN Experience.

Level II - 12 Months - Less than 3 Years of RN Experience.

Level III - 3 Years - Less than 8 Years of RN Experience.

Level IV - 8 Years of RN Experience.

  • Participates in and maintains awareness of performance improvement initiates.

  • Conscientiously evaluates and integrates evidence-based practice and research into clinical practice.

  • Conducts and documents accurate clinical assessments.

  • Develops, implements, and evaluates the patient’s individualized plan of care.

  • Uses professional nursing judgment to individualize the plan of care based on assessment of the patient's needs.

  • Manages own patient assignment by delegating and effectively based on patient needs and skills of team members.

  • Develops a comprehensive educational plan for the patient and family, utilizing appropriate resources, and documents according to department standards.

  • Recognizes patient/family rights through support of their informed decisions and advanced directives.

  • Continually evaluates and revises the care plan according to changes in the patient’s health status.

  • Develops a comprehensive discharge plan utilizing appropriate resources and referrals including community resources.

  • Supports the development of new staff members and helps facilitate their transition into unit. 

  • Guides the professional growth of department staff through precepting.

  • Participates in professional development by identifying learning needs and seeking appropriate education.

  • Participates in departmental committees as needed.

Job Description

Minimum Qualifications:
1. Massachusetts RN Licensure.

2. 12 Months - Less than 3 Years of RN Experience.

3. Current Basic Life Support (BLS) Certification.

4. Department Specific: Certification(s) relevant to the department and the type of patient care being provided may be required.

Preferred Qualifications:
1. Bachelor of Science in Nursing (BSN).

Physical Requirements: 
1. Occasional lifting of 30-35 lbs and turning patients.

2. Prolonged standing and walking.

3. Occasional twisting, bending, reaching, pushing/pulling, sitting, kneeling, and squatting.

4. Exposure to infectious disease and possible carcinogenic chemicals.

5. Exposure of blood-borne pathogens.

6. Requires fine manual dexterity to lift and maneuver patients.

7. Requires visual and aural acuity necessary to monitor patient care.

Skills & Abilities:
1. Ability to effectively communicate with patients, families, physicians, and healthcare team.

2. Knowledge of and application of the nursing process.

Th is position works closely with leadership and a broad spectrum of healthcare providers and administrative personnel to ensure optimal patient care in the satellite clinical setting . This position is housed off-site in a n affiliated organizational setting with the potential to flex on-site as needed to meet patient care needs . This position works in close liaison with the Health Care Team to ensure that patient care is coordinated, comprehensive, and that care transitions are seamless . This position practices at the of their scope and actively develops, coordinates, oversees and administers care to patients in accordance within the framework of team-based patient-centered model of care . Job Description Minimum Qualifications : 1. Bachelor’s degree in Nursing (BSN) . Associates degree considered with review of Nurse Executive Council . 2. Registered Nurse (RN) license. 3. Basic Life Support (BLS) certification. Preferred Qualifications : 1. Develops and maintains collaborative relationships between all members of the Health Care Team inclusive of administrative, clinical, research and inpatient partners thus producing high performing, patient focused teamwork. 3. Acts as a resource for all staff in regards to education, planning, relocating and development of evidence-based, best practice clinical programs pertaining to the specialty or primary care patient population. 4. Mentors and actively participates in overseeing all clinical support staffing in the ambulatory clinical setting; delegates responsibilities appropriately and within scope . 5. In conjunction with satellite clinical and administrative leadership, participates in coordinating any transition of patient care from the satellite setting to another in collaboration with and maintaining open communication with a wide range of interdisciplinary team members with the goal of achieving an exceptional patient experience. 6. Identifies high risk patients and works with the appropriate services and interdisciplinary team members to achieve the best possible patient care and outcomes. 7. Promotes shared decision making with patients in regards to healthcare; educates the patient and /or family regarding the patient’s clinical condition, treatment, and their role in their healthcare decisions. 8. Utilizes a variety of telecommunications technologies to assess, triage, provide nursing consultation, and perform follow-up and surveillance of patient status and outcomes . 9. Collaborates and actively engages with the care team and practice leadership to identify opportunities and solutions to improve quality, patient access and cost savings. 10. Demonstrates compliance with regulatory requirements i.e. MIPS, fall assessment . 11. Participates in satellite quality and safety initiatives as indicated . Physical Requirements : 1. Location is off-site in an affiliated satellite location . 2. Frequent travel may be required from satellite clinic to main hospital . 2. Ability to oversee multiple issues/ situations simultaneously with an emphasis on safety . 3. Prolonged, extensive, or considerable standing/walking . 4. Lifts, positions, pushes and/or transfer patients and equipment . 5. Considerable reaching, stooping, bending, kneeling, crouching . 6. Frequent exposure to hazardous chemicals, sick patients, bodily substances, noise and possible exposure to radiation, lasers, electric shock, etc 7.
