Job Summary
 

The Nurse Practitioner is responsible for meeting all clinical, administrative, and financial performance metrics. This position is responsible for implementing, delivering, and ensuring that the highest quality of patient-centered care is delivered.

Job Responsibilities

  • Provides patient centered, comprehensive primary care services

  • Participate in the selection of suitable treatment plans for all patients

  • Supervise staff to ensure the care team is providing appropriate care in accordance with their duties and obligations

  • Assist other care providers and extended care teams

  • Provide continuous availability for medical and technical questions to the employees, patient care staff, and ancillary care providers including after hours coverage

  • Actively participate in quality improvement programs, as well as participate in multidisciplinary quality assurance programs

  • Collaborate with the management team as needed to ensure improvement of patient care

  • Cooperate with clinical outcome reviews and measurements 

  • Comply with all policies, procedures, and protocols

  • Provide general medical care and treatment to patients in the health center under the direction of a physician (depending on state)

  • Submits health care plan and goals of individual patients for periodic review and evaluation by physician.

  • Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures

  • May refer patients to physician for consultation or to specialized health resources for treatment.

  • Performs other duties as assigned.

Required Qualifications 

  • Minimum of 5 years of clinical experience in Internal Medicine or Family Practice

  • Must be Board Certified

  • Must be licensed to practice in appropriate state

  • BLS (Basic Life Support) certification required

Preferred Qualifications

  • Proficient in promoting the clinic

  • Strong Communication skills 

  • Patient communication skills 

Physical Job Requirements

  • May require standing, walking and sitting for extended amounts of time.

  • Occasionally lift and carry items weighing up to 50 lbs.

  • Manual and finger dexterity and hand-eye coordination

  • Includes full range of body motion including potential of handling and lifting patients.

  • Requires corrected vision, hearing and speech within normal ranges. Must be able to effectively communicate with patients and team members.

Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided. Complies with applicable laws and regulations with respect to Collaborative Agreements. Initiates reimbursement for services rendered. Assists patients and families with goal setting related to treatment options and advanced care planning. Provides training and continuing education for staff. Assists in development of clinical practice guidelines/standards in support of quality palliative care. Adheres to the practice of confidentiality regarding patients, families, staff and the organization. Collaborates/Communicates with Medical Director, attending physician, DCS, and IDT: Collaborates/ Communicates with IDT needs of patient for clinical and psycho-social interventions. Assists in identifying the need for intervention of other IDT members. Effectively communicates patient and family needs to IDT. Completes and submits required paper work within 24 hours of visit completion. Educates patient and caregiver regarding: Care of patient. Disease process. Dying Process. Symptom control. Wound care. Performs other duties as assigned. If providing Hospice Face to Face Visits: Performs routine and emergency assessments: Completes routine and emergency recertification visits on each patient entering their 3rd benefit period or beyond (including admissions and transfers). Records observations, treatments, and other pertinent information. Communicates with Medical Director, Attending Physician, DCS and IDT as directed regarding findings from the visits and any measures to alleviate symptoms. Provides physical and emotional care to support the patient and family in period of crisis. Performs other duties as assigned. If providing Palliative Care: Performs routine and urgent assessments: Consistent communication of availability to manager. A minimum of 4 days of availability per month is required, however a minimum number of visits per month is not guaranteed to the PPV NP. Ability to triage consults by patient acuity. Performs bedside palliative care with patient/family education. Orders, performs, and interprets laboratory and radiology tests within scope of professional practice. Prescribes medications including controlled substances to the extent delegated and licensed. Orders treatments and durable medical equipment as indicated. Performs other therapeutic measures as indicated. Collaborates/Communicates with Medical Director, attending Physician, hospital staff and IDT: Participates in monthly IDT meeting with market team. Collaborates/ Communicates with IDT needs of patient for clinical and psycho-social interventions. Assists in identifying the need for intervention of other IDT members. Effectively communicates patient and family needs to IDT. Completes and submits required clinical documentation within 24 hours of visit completion. Educates patient and caregiver regarding: Care of patient. Disease process. Goal setting. Symptom control. Treatment options. Prognosis. Advance Care Planning. Supervises nurse trainees, including graduate and advance practice nurses in field placements. Participates in supervision of other trainees, including medical students, residents, fellows, and social work and psychology trainees. Performs other duties as assigned. Education and/or Experience Master’s degree in Nursing required. If providing Hospice Care: Minimum of one (1) year nursing practice or equivalent experience required; hospice, oncology, or home health and/or long-term care preferred. If providing Palliative Care: Minimum of five (5) years nursing experience required; clinical nursing care in oncology, hospice, geriatrics, intensive care preferred. Skills Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage. Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications. Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces hospice philosophy. Strong organizational and time management skills. Must be able to work a flexible schedule to include nights and weekends. Must be able to travel. Ability to maintain confidentiality of information, such as patient and Company files. Certifications, Licenses, and Registrations Active and unencumbered CNS or Nurse Practitioner license in state of employment required. Certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric required. Active NPI number. Maintain active DEA certification if state allows prescription of controlled substances by APPs. Board Certification in Hospice and Palliative Care is preferred, but not required. State Specific Requirements Other All states except Iowa: Current collaborative physician agreement in place. Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions.
