Responsibilities

Complete client assessments and reassessments
Assist with care planning and documentation
Monitor client health and communicate changes
Provide nursing services within the LPN scope of practice
Support caregiver training as needed
Communicate professionally with clients, families, caregivers, and office staff

Qualifications

Current Ohio LPN license in good standing
Current CPR certification
Valid driver's license, reliable transportation, and auto insurance
Strong communication and organizational skills
Home care experience is preferred but not required
