Summary We are looking for applicants with 3+ years of experience. The Urgent Care APP is a member of the clinical practice team and works collaboratively with other staff members to provide high quality patient care according to the PCMH model. Urgent Care Medicine APPs at Kennedy Community Health will have the opportunity to work in partnership with the numerous other disciplines offered at the health center including mental and behavioral health, pharmacy, Medication Assisted Treatment (MAT) program, Reproductive Health, Gender Affirming Care, Dental and Optometry. Clinical Responsibilities : Provides medical care to patients with acute medical problems or acute exacerbations of chronic illnesses. Examines patients, formulates diagnostic plans, and orders diagnostic testing. Interprets examination findings and test results and implements treatment plans. Provides acute care for patients across the entire lifespan, including infants and pregnant patients. Determines the need for consultation and assists in medical care and treatment provided by other specialists. Participates in other care procedures according to training and demonstrated ability. Prescribes and assesses effectiveness of pharmaceuticals, other medications and treatment regimens as appropriate for patient’s medical conditions. Promotes continuity for patients with the primary care team within EMK and facilitates appropriate follow up with primary care team when appropriate Meets the Health Center’s annual productivity goal. Maintains and updates Electronic Health Records (EHR) in a systematic format and completes required documentation within 48 hours. Our schedule includes weekday evening hours and Saturday morning hours on a rotating basis. Required Qualifications Completed Master’s level Nursing Practice Degree and Family Nurse Practitioner Certification FNP Board Certification Active, Unrestricted FNP License in Massachusetts. Summary To provide comprehensive services to psychiatric patients in an ambulatory setting and coordinate care of patients together with supervising physician(s). The Psychiatric Nurse Practitioner is a member of the clinical practice team and works collaboratively with other team members in providing patient care. As part of the team-based approach to care, the Psychiatric Nurse Practitioner will participate in coordinating care for individual patients, will support patients and families in self-management, through evidenced-based approaches as may be appropriate, will engage in health center projects that are aimed at specific patient population needs, and will utilize effective and appropriate communication strategies, such as health literacy approaches when working with patients. Essential Duties & Responsibilities 1 Provides psychiatric care to children, adolescents, and adults with acute psychiatric problems. Provides Preventive Health Services according to Practice Standards. 2 Provides telehealth services to patients. 3 Evaluates, diagnoses, treats and makes appropriate use of diagnostic testing and consultation according to delineated clinical privileges and approved MH Clinical Protocols. 4 Collaborates care with primary care providers of EMKCHC patients. 5 Collaborates care and referral to other specialty medical and mental health services. 6 Consults or refers patients with complex or dynamic medical problems to their physician partner. Required Qualifications: Active, Unrestricted License in Massachusetts as an Advanced Practice Nurse. Active, Unrestricted Controlled Substance Number in Massachusetts 3+ Years of prior experience working with Children and Adolescents as a Psychiatric Nurse Practitioner.
