Develop and implement individualized care plans based on current and prior medical history
Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
Administer physician-ordered medications and treatments with skill and empathy
Educate patients and their families on care techniques and symptom management
Track and communicate changes in patient condition, ensuring timely and effective responses
Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided
Collaborate with interdisciplinary teams to ensure comprehensive care delivery
And contribute to other supportive duties as part of a compassionate, mission-driven team
And additional duties as assigned

Uses health assessment data to determine nursing diagnosis. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions, including the patient and family in the planning process. Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient as appropriate per assessment and plan. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Teaches the patient and family/caregiver self-care techniques as appropriate, provides medication, diet, and other instructions as ordered by the physician, and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Provides and maintains a safe environment for the patient. Assists the patient and family/caregiver and other team members in providing continuity of care. POSITION QUALIFICATIONS Graduate of an accredited school of nursing Registered Nurse, with a License in good standing to practice Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing and minimum of two (2) years' experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. Management experience not required. Responsible for supervising hospice aides. Once an offer of employment is made, it is contingent upon satisfactory references, as requested, and criminal background checks by regulation.
