The Administrator RN I provides strategic leadership and operational oversight for assigned Hospice location with a developing census. This role will oversee and lead the day-to-day operations, including managing and directing interdisciplinary teams, ensuring the delivery of high-quality and patient-centered care, fostering a culture of compassion, respect and integrity to meet business and organizational goals, regulatory standards, and community needs. Responsibilities include leadership and development, financial performance, community outreach, human resources, office operations and communication and relationship building with the Governing Body, referral partners, the community. In this role, the Administrator ensures assigned Hospice location delivers outstanding service, while upholding the company’s mission, vision and values.

Essential Functions

  • Provides hands-on leadership for a hospice program with growth opportunity, accountable for clinical quality, operational performance, and sustainable census expansion.

  • Promotes/provides for core orientation plus location and job specific orientation, in service training and continuing education for employees, volunteers and contract personnel.

  • Recommends a sufficient number of qualified and competent persons to provide services.

  • Responsible for the selection, supervision, and evaluation of interdisciplinary team members.

  • Collaborates with the Governing Body, Corporate leadership and employees to establish goals and standards for performance; reviews and monitors progress regularly and redirects efforts where goals are not being met.

  • Determines qualifications and competence of employees.

  • Completes all performance evaluations prior to due date.

  • Takes appropriate and timely action in disciplinary situations; documents actions taken in accordance with policy.

  • Makes appropriate assignments among employees in accordance with their personal abilities and limitations; ensures fair and consistent workload distribution.

  • Manages Office Operations effectively

  • Directs the multiple teams of the assigned location, if applicable.

  • Plans and organizes tasks and resources to achieve strategic goals and objectives.

  • Participates in selecting outside sources for needed services.

  • Ensures contract management is maintained for services from outside vendors/providers.

  • Assesses and improves branch/department performance as well as individual team's performance.

  • Ensures documentation of services is accurate and timely.

  • Monitors and supervises quality of patients' care of assigned location(s).

  • Processes emergency calls and requests for assistance.

  • Assists with problem solving for patients in all settings

  • Prepares, monitors, and reviews monthly bills, supply costs, expense requests, mileage reimbursement, payroll and other expenses.

  • Makes recommendations for service area regarding market opportunities for new programs or service.

  • Ensures accuracy of marketing materials, public information materials and activities.

  • Provides a safe environment for patients, visitors, customers and employees.

  • Investigates and completes detailed reports of any incidents/accidents, grievances, complaints, medication errors and other problems within the time frames established by policy, and implements corrective and/or preventive action immediately.

  • Works with physicians, patients, families, staff and others regarding problems, needs and concerns.

  • Responsible for supervision and evaluation of family satisfaction surveys.

  • Incorporates safety topics into staff meetings.

  • Regularly reviews safety policies and procedures (to include the Emergency Preparedness Plan); ensures that all employees are oriented to and understand their role in safety policies and procedures.

  • Manages Financial Resources prudently and effectively.

  • Completes financial reviews on recommended schedule.

  • Budget worksheets are completed and submitted by scheduled deadlines.

  • Annual expenditures are within annual budget except where variances are volume driven and have been approved.

  • Resource needs are anticipated and budgets are planned to meet those needs within known parameters.

  • Budget variances are justified on a regularly scheduled basis.

  • Continually seeks ways to reduce costs without affecting quality.

  • Controls use of overtime and temporary personnel except when justified by increased volume or decreased staffing levels.

  • Recommends space and other resources for needed services.

  • May be asked to perform routine visits to patient homes or institutional setting and/or participate in on-call schedule.

About You

  • Education:

  • Registered Nurse with minimum of Associate’s Degree in Nursing required

  • Bachelor’s degree in nursing (BSN) from an accredited school of nursing preferred.

  • Experience:

  • Three to five years’ healthcare sales/ management experience preferred.

  • Minimum one year management/supervisory experience required.

  • Two or more years’ experience in hospice/home care required

  • Meets all state-specific requirements for the Administrator role.

  • Three years’ experience in direct patient care in a clinical setting preferred.

  • Strong understanding of hospice regulations, compliance, and Medicare conditions of participation

  • Proven ability to manage teams, budgets, and interdisciplinary operations .

  • Exceptional communication, organizational, and leadership skills

  • Commitment to high-quality, compassionate care for patients and families.

This Nursing schedule includes; Casual/On Call Day or Evening shifts Must pick up three shifts a month; two being weekend shifts Responsibilities Maintains current assessment and documentation of resident health status: Performs initial and on-going assessments per the applicable Minnesota Assisted Living regulations and Nurse Practice Act. Ensures assessments are completed every 90 days per regulatory requirements. Works with resident and/or responsible party through collaboration, education, and timely communication to ensure appropriate service and placement. Service Agreements: assure that resident has an up to date service agreement at time of admission, with any changes in services and annually. Develops, maintains, and implements individual emergency plans based on health status and Advance Care Planning documents, including POLST. The Fountains offers seniors 92 Independent Living, Assisted Living, and Memory Care apartments. The Fountains and their exceptional staff are known for creating a warm, friendly, fun community. This position is responsible for ensuring that the safety and health needs of the residents are met. Responsibilities Maintains current assessment and documentation of resident health status: Performs initial and on-going assessments per the applicable Minnesota Assisted Living regulations and Nurse Practice Act. Ensures assessments are completed every 90 days per regulatory requirements. Works with resident and/or responsible party through collaboration, education, and timely communication to ensure appropriate service and placement. Service Agreements: assure that resident has an up to date service agreement at time of admission, with any changes in services and annually. Develops, maintains, and implements individual emergency plans based on health status and Advance Care Planning documents, including POLST. Maintains clinical charts and progress notes using computerized documentation system. Communicates changes in resident conditions to medical and other relevant individuals and perform the appropriate follow up to health needs Coordinates resident health services: Facilitates coordination of services with internal and external providers. Assures that all physician orders are signed and implemented per the applicable Minnesota Assisted Living regulations. Works with resident and/or representative(s) to assure delivery of appropriate services and funding. Manages and oversees medication and treatment administration. Assures medications and supplies are ordered in a timely manner. Assures medication and treatment practitioner orders are renewed at least annually and as necessary when there is change in medication and/or treatment. Minnesota Assisted Living regulations will be followed in obtaining and implementing practitioner orders. Assures medi boxes, bubble packs, and/or syringes are filled per medication orders/changes for medication administration by designated staff. Assures that all medication administration procedures are correctly followed. Takes appropriate action with staff regarding any problems or issues. Assures accuracy of medication and treatment records, including documentation of administration and appropriate signatures. Monitors work and performance of licensed and unlicensed staff. Meets with staff to coordinate care and address care issues. Plans and assigns/delegates work to staff within established guidelines. Interviews, trains, and orients new employees as directed. Assists and supports staff to complete assignments as needed. Assists Director of Health Services (DHS) in monitoring and evaluation of staff performance. Schedules and completes Supervisory Visits. Complies with Federal, State and Local Laws. Assists DHS in scheduling activities that support efficient and effective utilization of staff as needed. Performs any other duties as delegated by Director of Health Services. Support DHS to act as RN on call during scheduled times. Answers calls and gathers information related to assisted living community residents health and safety. Takes action as appropriate to the situation. Work collaboratively with indentified internal and external support providers. Actions taken may require phone directives and/or traveling to the assisted living community to resolve the situation.
