Overview

The Behavioral Health RN collaborates with the care coordination team to facilitate referrals to appropriate facilities and next levels of care. This role supports discharge and transition planning, coordinates final disposition arrangements, and ensures patients receive timely access to continued treatment and support services. 

Responsibilities

  • Monitor repeat and high-risk behavioral health patients to facilitate appropriate care management and continuity of services.
  • Maintain current knowledge of regulatory requirements, accreditation standards, and best practices related to discharge planning and behavioral health patient transitions.
  • Utilize telepsychiatry technology, complete required transfer documentation, and ensure accurate and timely patient record documentation.
  • Collaborate with nursing staff, physicians, and interdisciplinary care team members to assess patient needs, develop discharge plans, implement interventions, and evaluate outcomes.
  • Serve as a behavioral health resource by maintaining and updating referral resources, community services information, and unit-based educational materials.
  • Support behavioral health patient care initiatives across all units through staff education, training, and consultation in partnership with department leaders.
  • Coordinate referrals to community-based organizations, behavioral health providers, and local support agencies to ensure appropriate patient follow-up and continuity of care.
  • Collaborate with state agencies and external partners to facilitate patient placement, treatment access, and discharge planning needs.
  • Communicate effectively with physicians, nursing staff, care coordinators, and other healthcare professionals regarding patient status, treatment plans, and discharge arrangements.
  • Provide patient and family education regarding treatment plans, available resources, discharge instructions, and ongoing behavioral health support services.


Qualifications

 

  • 1 year of behavioral health experience
  • Certification for Delaware Committal Process preferred

  • Ability to use resources effectively to insure timely transfer/discharge of patients to the appropriate level of care. 

     

  • Ability to effectively assess patients holistically for behavioral health needs, work with physicians for plan of care, transfer and discharge planning needs, real and/or potential.
  • Ability to make sound decisions regarding outpatient resources in collaboration with medical provider.
  •  Ability to prioritize cases and complete tasks in a way to expedite discharges/transfers.
