Job Summary:
Provides assistance to the surgical team during the intra-operative phase of surgical care for the Northwest Surgical Services region. Works in collaboration with the surgeon and health care team in the performance of surgical procedures, including assessment, planning, directing, providing and evaluating advanced nursing care. All RNFAs will participate in a regional on-call rotation to provide surgical assistance for elective, emergent and urgent surgical cases.

Essential Responsibilities:

  • Representative Functions or Duties: Practice standards of performance: Pre-operative. Performs pre-operative assessment and evaluation. Collaborates with the surgical team on plan of care. Intra-operative: Provides knowledge based technical assistance to the surgeon during the operative procedure according to AORN standards, licensure, state regulation and facility/practice protocols, including: Using instruments and medical devices. Providing surgical exposure. Handling and/or cutting tissue. Providing hemostatis, clamp, cut, and ligate tissue as directed by the surgeon. Suturing as directed by the surgeon. Post-Operative. Evaluates the patient post-operatively according to facility protocols.

  • Assessment Standard of Performance & Planning: Communicates information so that continuity of care is maintained. Communicates with patient, family and other staff in a professional and courteous manner. Obtains and demonstrates the skills and knowledge needed to provide appropriate population based and culturally sensitive care to patients and families through education, training and experience.  Provides continuous reassessment through the intra operative period. Utilizes knowledge of type of procedure to plan for appropriate set up.

  • Practice standards of performance: Function as an assistant in all surgical specialties at the direction of the attending surgeon. Performs RNFA responsibilities in accordance with the Nurse Practice Act of the State of Oregon, institutional policies and procedures of Kaiser Permanente and the AORN Position Statement on RN First Assistants. Practice within limitations of preparation and experience. Demonstrate RNFA specific clinical skills. Review and update skills checklist on an ongoing basis as specified by policies of Kaiser Permanente. Intra operative nursing behaviors include:  handling tissue, providing exposure, using instruments, clamp, cut, and legate as directed by the surgeon, providing homeostasis, suturing as directed by the surgeon. Assists with room preparation in collaboration with the Circulating Nurse and scrub person. Brings patient X-rays into room NOTE: Notifies Charge Nurse or designee if films are not available. Assists in patient transport to the operating room and patient transfer to the operating room table. Provides assistance to Anesthesia during induction. Participates in pre-op briefing time out process. Assists in patient position. Assists in patient preparation, i.e., prepping, catheterization, shaving, as necessary. Assists in patient draping. Sets up ESU, lights, suction for the sterile field. Monitors free counted items on the sterile field. Observes for breaks in sterile technique and takes appropriate action as necessary.

  • Assists with instrumentation and equipment as requested. Applies dressings and secures drainage tubes. Assists with patient transfer and positioning post operatively. Assists with room tear down and turn over in collaboration with the Circulating Nurse and scrub person. May perform pre and post-operative patient education as designated. Review chart for pertinent data. Assist in maintaining a safe environment and promoting efficiency in surgical. Procedures.

Job Summary: The Registered Nurse – Recuperative Care (RN-RC) provides admission screening and intake, care planning, case management, patient education and health navigation, and clinical care within the scope of practice, for patients being discharged from the hospital setting without stable housing for recovery. This position works under the direction of a VG Medical Director with a non-clinical employee to visit patients assigned to 10 beds within a larger homeless shelter located in Hillsboro for basic medical supervision and care coordination. The Recuperative Care Program (RCP) is a new program in partnership with Washington County Supportive Housing Services and Greater Good Northwest to provide acute and post-acute care specifically for people experiencing homelessness who are too ill or frail to recover from an illness or injury on the streets or in emergency shelter, but who do not require hospital-level care or skilled nursing services. This position will be based out of the VG 7th Ave clinic and will require travel to the RCP program site in Hillsboro and may require travel to local hospitals for discharge planning. Essential Duties and Responsibilities: · Screen referrals from hospitals and coordinate with discharge planners to assess patient acuity, program requirements, and bed availability to determine appropriateness for RCP admission. · Identify potential clinical and supportive service needs while within respite program. · Proactively communicate with RCP participant’s primary care team (VG or external) to facilitate early post-hospital contact. · Obtain required consents and releases for coordination of care. · Develop care plan based on patient goals and the reason for admission to RCP. · Gather information for a clinical care plan including physical and psychological history and assessment; list of current medications, dosages, and allergies; vital signs; other pertinent information, such as laboratory or other test results. · Identify discharge indicators and develop a discharge plan with patient. · Participate in and facilitate multi-disciplinary care coordination meetings with appropriate providers (e.g., PCP, home health, pharmacies, specialty care). · Coordinate with shelter case management to address SDOH and other non-clinical needs, · Provide shelter staff education and support around basic medical knowledge as relevant for evening and weekend supervision of RCP residents. · Huddle daily with RCP team to create and implement daily action plans, performing transition of care visits for patients being discharged from hospitals, following up with outside provider recommendations after outpatient visits and ensuring that treatment recommendations and medication changes are acknowledged and actualized. · Facilitate patient access to obtain durable medical equipment (DME), wound care, oxygen, and incontinence supplies (as needed). · Provide patient education and health coaching on topics including diabetes, hypertension, and other chronic diseases, including assisting patients with self-management goal setting. · Educate patients on how to utilize after-hours number or daytime nurse triage to address immediate clinical needs and provide guidelines for when to contact RCP team or PCP vs. seeking emergency department care. · Log vital signs and monitor for changes in health status as indicated by the care plan. · Administer pre- and post-procedural or operative care, including minor wound care per discharge instructions, monitor for changes in health, and recommend follow-up services as needed. · Support self-administration of medications and patient progress towards self-management of medications. Coordinate with clinical pharmacy where appropriate. · Assist with prescription management and reconciliation in conjunction with clinical pharmacy where appropriate. Position allows for a hybrid schedule of four (4) ten-hour days per week with remote work up to three (3) days per week, negotiated based on performance and successful completion of the 150-day probationary period and orientation. Remote work is expected during interruptions to clinic services due to inclement weather or other emergencies. Hybrid or remote work requires adequate Wi-Fi and private space to care for patients and properly store any confidential materials. Laptop and peripheral equipment will be provided. Essential Duties and Responsibilities: • Provide triage assessment over the phone utilizing the nursing process, making both independent and collaborative decisions to advise patients. • Utilize telephone interpretation services to meet the needs of patients where appropriate. • Provide high-quality customer service and a clinically appropriate and timely experience for patients calling the VGMHC Call Center. • Coordinate care services for follow-up with the Emergency Department (ED) and internal and external departments/clinics based on patient needs. • Perform patient education, including explaining to the patient the triage disposition, treatment plan (including use of medications and their effects and side effects if appropriate), infection control (if indicated), and preventative techniques to lower the risk of future disease or accident. • Offer patients guidance on self-care, medication management, and preventative measures based on clinical protocols. • Identify care gaps and problem-solve with patients to schedule appropriate appointments with their care team. • Update on-call providers as needed on the changing needs of complex, high-risk patients. • Document all interactions with patients and others involved in the care of the patient, accurately, timely, and efficiently in the Electronic Health Record (EHR). • Communicate problems or concerns immediately to the supervisor to prevent delayed or inappropriate care decisions. • Maintain up-to-date knowledge and skills in professional, clinical, and system VGMHC areas. • Perform other duties as assigned. • Handle protected health information (PHI) in a manner consistent with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) HIPAA Requirements: The CCAN will use PHI to answer patient questions about their medical problems or plan of care, to triage and schedule appointments for urgent medical problems, to relay information from the health care providers to the patient. Applying the minimum necessary rules of HIPAA, the designated record sets to which this employee will have access include scheduling, demographics, and view only patient account information in the practice management system, and the complete medical record, including confidential sections. Knowledge, Skills and Abilities Required: • Relevant nursing experience that utilizes the nursing process and assessment skills, in person or over the phone. • Strong ability to accurately elicit information, determine existing health problems and evaluate current symptoms without the advantage of a physical patient assessment required. • Strong critical thinking skills and clinical judgment. Flexible and adaptable to daily needs. • Proficiency in English, written and spoken language required. Proficiency in Spanish (or another 2nd language on the VG language list) preferred. Education and Experience Required: • Current Oregon RN License • Two (2) years’ minimum experience with telephone or in-person nurse triage required, 3-5 years of experience preferred.
