Based on the NMH framework for practice, the registered professional clinical nurse (RN) is accountable and responsible for the provision and coordination of clinically competent, evidence-based care for an assigned group of patients that includes diagnosis, outcome identification, planning, implementation and evaluation. Responsibilities:

The role of the professional clinical nurse is unique and essential to the care of the patient. The nurse demonstrates that unique and essential function through the framework for nursing practice.

Qualifications

Required:

  • Graduate of an NLN accredited school/college of nursing
  • Baccalaureate degree in Nursing or higher
  • Current State of Illinois, Registered Professional Nurse license
  • Basic Life Support (BLS) CPR through American Heart Association.  
  • Analytic ability (conceptual and quantitative)
  • Oral, listening and written communication skills
  • Strong human relation skills to interact effectively and collaborate with others in difficult situations
  • Use of a keyboard to enter or retrieve data

Preferred:

  • Experience in specific clinical area
  • Nursing specialty certification, if eligible

This position is a 100% onsite opportunity. You will need to be based in the greater Chicagoland area. UChicago Medicine Flossmoor offers specialty and primary care for the whole family. Our teams care for children, teens, adults and senior citizens who need annual wellness exams, school physicals, immunizations, injury treatment or help managing chronic illnesses, including diabetes. A Registered Nurse, UCM Care Network, is responsible for providing professional nursing care for patients including assessment, treatment, care planning, and medical care evaluation by following established standards, protocols and practices. Assists physicians and staff in coordinating patient care. Functions as primary liaison between patients, staff and physicians. This position will be located in a community-based physician practice. Essential Job Functions Obtains patient consent for care and ensures patient confidentiality Obtains and documents health history, information on chief complaint, vital signs, and health risk factors and assesses disease condition Provides patient care based on practice guidelines, standards of care, and federal/state laws and regulations Executes prescribed treatments and medical interventions, administers prescribed medications, and monitors and documents treatment progress and patient response Delegates as allowed under the Nurse Practice Act. Refers to/consults with physicians, other health providers, and community resources to prevent/resolve problems or concerns Educates patients and families about health status, health maintenance, and management of acute and chronic conditions Documents patient assessment and intervention data using established medical record forms/automated systems and documentation practices Participates in multidisciplinary teams to improve patient care processes and outcomes Tracks quality assurance data and monitors for acute and chronic care management Maintains/reviews patient records, charts and other pertinent information Posts tests and examination results Responds to/refers incoming patient treatment-related phone calls Maintains patient confidentiality Performs other duties assigned Required Qualifications Must be an actively licensed Registered Nurse in the state of Illinois Minimum of one year professional nursing experience Completion of nursing training from an accredited school of nursing Active BLS certification from the American Heart Association (AHA) Preferred Qualifications A Bachelor's degree in Nursing Prior clinic experience Position Details Job Type/FTE: Full-Time 1.0 FTE Shift: Days - 8 am – 5 pm Work Location: Onsite - Flossmoor Unit/Department: Endocrinology CBA Code: Non-Union
