About the Role:

As a Nurse Practitioner or Physician Assistant, you'll play a vital role in our comprehensive preventative care program, incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to prioritize proactive measures for enhanced physical and mental health.
