Position Overview The Palliative Care Nurse Practitioner provides comprehensive in-home medical care for patients with serious illnesses. This role focuses on symptom management, chronic disease support, advance care planning, and coordination with the patient's existing healthcare providers to ensure care aligns with each patient's goals, preferences, and quality-of-life priorities. Working independently in the field, the NP delivers evidence-based care while serving as a trusted clinical resource for patients, caregivers, and healthcare partners. What You'll Do Clinical Care Conduct comprehensive in-home assessments, including physical, psychosocial, and functional evaluations. Develop, implement, and adjust individualized care plans focused on symptom management and quality of life. Evaluate and manage pain, shortness of breath, nausea, anxiety, fatigue, and other complex symptoms. Prescribe medications, order diagnostic testing, and provide clinical interventions within your scope of practice. Monitor patient progress and adjust treatment plans in collaboration with physicians and interdisciplinary partners. Participate in occasional on-call responsibilities as needed. Care Coordination Collaborate closely with physicians, specialists, home health, hospice, social workers, and other community partners. Preferred Qualifications Experience in palliative care, hospice, geriatrics, internal medicine, family practice, or chronic disease management. MAJOR FUNCTIONS: Under general direction, the Nurse Practitioner is responsible under direction of a physician for providing general medical care, preventative care and treatment to patients in one of the agency’s clinics.
