RN Patient Navigator - Dialysis
Temple, TX | Full-Time | Monday-Friday, 8:00 AM - 5:00 PM

Join Baylor Scott & White Health's growing Outpatient Dialysis team supporting approximately 500 patients across three dialysis units. The RN Patient Navigator serves as a patient educator and advocate, helping patients successfully navigate their dialysis journey while supporting quality and patient experience initiatives.

Key Responsibilities

Conduct new patient orientation and dialysis modality education.
Provide targeted monthly patient education.
Partner with leadership on quality improvement and patient experience projects.
Build strong relationships with patients to improve outcomes.

Job Summary

As a Patient Navigator RN, ensure smooth patient care coordination. Assess patient needs and help plan, implement, and evaluate healthcare with the medical team. Work autonomously and with the team. Also serve as advocate, teacher, and coordinator for patients.

Essential Functions of the Role

  • Care for patients in all aspects by evaluating their health based on doctor's orders and creating a healthcare plan.
  • Schedule outpatient tests for diagnosis, stay in touch with the healthcare team.
  • Visit doctors often, support patient-office communication.
  • Collaborate with healthcare professionals for optimal patient care.
  • Conduct follow-ups every 3 months and 1 year.
  • Maintain up-to-the-minute and comprehensive nursing records, also updating patient details in the Patient Navigator database.
  • Attend case conferences to expand your knowledge. Participate in community service events.
  • Work together on improvement projects to enhance service for patients, families, and the healthcare system.
  • Welcome patients and families, offer facility details, directions, and general information.
  • Assist in guiding patients and families through the hospital, especially to hard-to-find places.

  • Proficiency in aiding patients to access resources within the community.
  • Collaborative skills to work in tandem with both internal and external medical personnel, partnerships, and organizations.
  • Adaptability to assess, adjust, and respond calmly to changes and crisis situations.
  • Discretion in maintaining patient confidentiality in accordance with all relevant policies and regulations.

  • A Bachelor's degree.
  • At least 3 years of relevant experience to the table.
  • Registered Licensed Nurse.
  • You have, or are willing to obtain within your first 30 days, a Basic Life Support (BLS)

Be the Best Hospice After Hours Admission RN You Can Be If you meet these qualifications, we would love to meet you: Current licensure as a Registered Nurse in the applicable state is required, with an associate degree preferred. CPR certification is recommended (required for direct patient care in Georgia), along with prior experience in hospice, home health, medical-surgical, or population-specific nursing such as geriatrics or pediatrics. Must demonstrate a willingness to participate in an on-call rotation and maintain reliable transportation, valid driver’s license, and state-required liability auto insurance. Strong communication and organizational skills are essential, along with proficiency in Microsoft Office Suite and EMR systems. The ability to collaborate effectively within an interdisciplinary team is required. This role involves regular physical activity, including lifting patients over 200 lbs with assistance, transporting supplies and equipment up to 20 lbs for distances of 500 feet, and frequent standing, walking, sitting, stooping, and reaching. Responsibilities: Conducts comprehensive nursing assessments and collaborates with patients, families, physicians, and the Interdisciplinary Group (IDG) to develop, implement, and evaluate individualized plans of care in alignment with regulatory requirements and the Case Management Recipe. Maintains timely and accurate documentation, ensures care plans reflect current patient status, and communicates effectively with the care team to coordinate optimal use of resources, especially in complex or high-risk situations.
