Responsibilities:

Provide personalized care and assistance to patients in their homes, including but not limited to bathing, dressing, grooming, and toileting
Assist with daily living activities, such as meal preparation, light housekeeping, and medication reminders
Monitor and report changes in patient condition to the Nurse, Physician, and family members
Maintain accurate and detailed records of patient care and progress
Collaborate with our in-office coordinator and field case manager to ensure seamless care coordination
Participate in regular team meetings and training sessions to stay up-to-date on the latest best practices and industry developments
