Responsibilities

Provide compassionate and skilled care to patients in their homes, including assistance with activities of daily living (ADLs), personal care, and homemaking tasks
Develop and implement personalized care plans in collaboration with patients, families, and healthcare teams
Monitor and report patient progress to the healthcare team, ensuring continuity of care and effective communication

Requirements

High school diploma or equivalent required
Completion of a recognized Home Health Aide training program
Current certification as a Home Health Aide (HHA) or willingness to obtain within a specified timeframe
Ability to work independently and as part of a team
Strong communication, empathy, and problem-solving skills
Reliable transportation and ability to travel to patient homes
