    GENERAL DUTIES


    This job description is intended to describe the general nature and level of work performed by employees assigned to this department.     Patient Care: Provides quality care to patients. Promptly responds to patient calls and messages and ensures same day completion of calls and messages. Maintains accurate patient and business records including the completion of encounters. Observes patients for signs and symptoms of abuse/neglect and report immediately in accordance with policy. Recognizes emergencies and responds appropriately in adherence to organizational policy and procedure. Coordinates plans of care, periodically review these plans and implement methods to keep the quality of care under constant surveillance.

    Compliance: Stays abreast of all other responsibilities required as set forth in any federal and state laws, statutes, or regulations as enacted or as may be enacted or amended. Maintains the confidentiality of all patient information as established by the facility’s HIPAA policies and procedures. Maintains the required continuing education hours to assure continued competence.

    Professionalism and Support: Demonstrates professional ownership and commitment to the principles and core values of guest relations. Demonstrate leadership of the clinical team. Communicates with clinic leadership, escalate issues and present solutions.

    SAFETY AND INFECTION CONTROL STANDARDS: Assists with compliance to safety, environmental, infection control, quality improvement and other regulatory standards.Implements standard and transmission-based precautions, as outlined in policies. Performs hand hygiene, as outlined in relevant policy. Ensures patient safety during clinic visits, in accordance with appropriate policy. Maintains a clean clinic working environment. Reports any safety hazards or violations in patient or clinic environment to clinic manager or designee.

    Collaboration: Provide input in hiring, disciplinary action and termination decisions of clinical personnel. Attends all daily huddles, staff meetings and provider meetings. Participates in on-going performance improvement activities and initiatives.

    EDUCATION QUALIFICATIONS

    Master's Degree from an approved NP Program required.

    Basic Life Support (BLS) from American Heart Association

    Registered Nursing License (RN) from the Louisiana State Board of Nursing

    Advanced Practice Registered Nurse (APRN) from the Louisiana State Board of Nursing

What You'll Do As the LVAD Coordinator, you will provide comprehensive inpatient and outpatient management for patients receiving ventricular assist device (VAD) therapy while serving as a key clinical and operational leader for the program. Program Highlights As a CTICU Lead APP, you will care for patients across a complex cardiac and thoracic surgical population, including: Open Heart Surgery Heart Transplant Lung Transplant Ventricular Assist Devices (VADs) Mechanical Circulatory Support Advanced Cardiothoracic Critical Care Schedule: Full-time 12-hour shifts Day/Night rotation Position Summary The Cardiothoracic ICU Lead APP serves as an advanced clinician and leader responsible for delivering comprehensive, evidence-based care to acute and critically ill patients while supporting operational excellence within the APP team. In addition to direct patient care responsibilities, the Lead APP provides guidance, mentorship, education, and support to fellow Advanced Practice Providers, helping ensure safe, effective, and high-quality care across the CTICU. Key Responsibilities Advanced Clinical Practice Assess and manage complex acute, critical, and chronically ill cardiothoracic patients. Identify urgent and emergent conditions through comprehensive clinical evaluation. Analyze physiologic and diagnostic data to develop and implement treatment plans. Perform advanced therapeutic interventions within approved scope of practice and credentialed privileges. Respond to changes in patient condition and modify treatment plans to optimize outcomes. Care Coordination & Patient Management Manage and document comprehensive plans of care. Facilitate transitions between levels of care and healthcare settings. Incorporate health promotion, injury prevention, and patient safety principles into patient care. Collaborate closely with physicians, nursing teams, pharmacists, therapists, and multidisciplinary team members. Leadership & Team Development Provide leadership, guidance, and support to inpatient APPs. Assist with onboarding, orientation, and training of newly hired APPs. Serve as a mentor and preceptor for APPs, nurses, students, and other healthcare professionals. Support implementation of departmental policies, procedures, and evidence-based practice initiatives. Educate APP colleagues and staff regarding policy updates and clinical best practices. Education & Quality Improvement Serve as an educational resource for patients, families, nursing staff, and providers. Participate in quality improvement, patient safety, and performance initiatives.
