Responsibilities:

Urgent care, primary care, occupational medicine, and telemedicine
Minor procedures including toenail removals and foreign body removals
Suturing and splinting
General orthopedic evaluations and management
X-ray review (with Radiologist over-read)
EKG interpretation
General dermatology, eye complaints, and GYN care
Treating patients of all ages
Emergency management skills in a fast-paced setting

Schedule:

Shifts determined approximately two months in advance

MainStreet Clinic Hours:

Monday – Friday: 8:00 a.m. – 8:00 p.m.
Saturday & Sunday: 9:00 a.m. – 4:00 p.m.

KidsStreet Clinic Hours (select locations only):

Monday – Friday: 8:00 a.m. – 8:00 p.m.
Saturday & Sunday: 9:00 a.m. – 9:00 p.m.

Hours are subject to change based on company needs, training, float coverage, etc.

Qualifications:

Family Nurse Practitioner Board Certification required
MSN or higher required
Excellent written and verbal communication skills
EMR and Microsoft Office experience preferred

Position Summary The Advanced Practice Registered Nurse collaborates with physicians and interdisciplinary teams to provide comprehensive palliative and hospice care in various settings. The APRN delivers expert clinical evaluation, treatment, and management of patients with serious or terminal illnesses. Responsibilities include assessing patient needs, facilitating goals of care discussions, prescribing treatments, and supporting transitions of care. The APRN plays an integral role in educating staff, families, and the broader medical community about palliative and hospice care. PRN hours M-F days to include some weekend availability if needed. Position located in the Gainesville, FL area and surrounding area Essential Functions Provide medical expertise in pain and symptom management, including terminal prognosis evaluation. Act as attending provider in hospice contract beds in collaboration with the hospice physician. Coordinate admissions and discharges for hospice contract beds, including review of terminal diagnoses. Assess patient conditions, develop care plans, implement interventions, and evaluate outcomes. Conduct patient and family assessments, ensuring readiness for transition to hospice or home settings. Perform physical exams, diagnostic testing, treatment, follow-up, and consultations. Prescribe and administer medications and other therapeutic interventions. Collaborate and consult with hospice/palliative physicians, attending physicians, and interdisciplinary team. Support patients and families in clarifying care goals and end-of-life decisions.
