How you make a difference

The Licensed Practical Nurse (LPN) plays a vital role in patient care by contributing to the nursing process of assessment, planning, implementation, and evaluation. Working under the supervision of RN(s), the LPN provides direction and guidance in patient education and activities that align with their education and demonstrated competencies.

Key Responsibilities

Under RN supervision, assesses patients' physical, psychological, and social dimensions in both the Health Care Unit and housing units, contributing to individualized treatment planning and consulting with RNs and staff as needed.
Implements treatment programs directed by Health Care Practitioners, administering medications, including IV therapy (if certified), and performing diagnostic tests according to established protocols and procedures.
Collaborates in controlled substance counts at the beginning of each shift, adhering to safety techniques, procedures, and standard precautions while responding to codes or health emergencies within standard guidelines.
Assists in medical or minor surgical procedures, implements nutrition and therapeutic diet plans, and documents nursing encounters using SOAP charting, ensuring effective communication with nursing staff, physicians, and other relevant personnel.
Attends mandatory staff meetings and training sessions, demonstrating a commitment to continuous professional development and adherence to policies and procedures in providing high-quality patient care.

Additional Details

We are looking for a full time LPN to join our clinical team at Macomb County Jail. This position works 7a-7:30p and EOW.

Qualifications & Requirements

Education

Graduate from an accredited school of nursing.

Experience

Prefer a minimum of one (1) year clinic experience

Licenses/Certifications

Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment.
Must be able to obtain and maintain CPR certification.

SUMMARY: The Community Licensed Practical Nurse (LPN) supplements the nursing care needs of participants as established in the Interdisciplinary Team (IDT) plan of care. The LPN assists the primary care providers in the collection of specific assessment data that is used by the IDT in the planning of care needs. The LPN performs acts, task or functions under the direction of the RN, physician, or nurse practitioner, and acts only within the scope of his/her authority to practice. SPECIFIC DUTIES AND FUNCTIONS: The LPN works under the direction of the RN or primary care providers in the collection of specific assessment data, upon the initial visit to PACE of SE Michigan and upon routine re-evaluations; this data is used by the Interdisciplinary Team to plan for and evaluate participant needs. The LPN works with the participant, the family and all members of the interdisciplinary team in implementing the participants’ plan of care. The LPN assists with data collection for routine as well as episodic visits to the clinic; the LPN communicates his/her findings to the medical provider or the team for evaluating or modifying the plan of care. The LPN prepares and administers medications as ordered by the physician he/she administers, in a safe, accurate, and timely manner. The LPN demonstrates knowledge of the medications he/she administers, including actions and side effects, and instructs the participant/family in safe administration of medications at home.
