Responsibilities:

Provide personalized care and support to clients in the comfort of their own homes, including assistance with daily living activities, medication management, and companionship
Develop and maintain positive relationships with clients, their families, and our staff to ensure a high level of care and communication
Maintain accurate records of care delivered and report any changes or concerns to our clinical team
Participate in ongoing training and education to stay current with industry best practices and regulatory requirements
Collaborate with our clinical team to develop and implement individualized care plans that meet each client's unique needs
