Responsibilities:

Provide in-home care to clients, assisting with daily living activities such as bathing, dressing, and grooming
Develop and implement personalized care plans tailored to each client's unique needs
Monitor and report changes in clients' conditions to supervising nurses and other healthcare professionals
Assist with light housekeeping, meal preparation, and managing medications as needed
Maintain accurate records of care provided and client interactions
Establish and maintain positive relationships with clients and their families
