Responsibilities

Provide exceptional care and support to clients, including assistance with daily living activities, medication management, and personal care
Develop and maintain positive relationships with clients, families, and care teams to ensure seamless care delivery
Conduct personalized care assessments and develop individualized care plans to meet client needs and goals
Administer medical treatments and services as prescribed by healthcare professionals
Monitor and report changes in client condition to healthcare professionals and care teams
Maintain accurate and detailed records of client care and services provided
