About the Role

As a Home Health Caregiver with SYNERGY HomeCare Of The West Valley, you will be responsible for delivering personalized care to our clients, including:

• Assisting clients with daily living activities such as bathing, dressing, and grooming
• Providing medication management and administration as prescribed by physicians
• Assisting with mobility and transfers, including feeding, toileting, and exercising
• Maintaining a safe and healthy environment for clients, including maintaining a clean and organized living space

Responsibilities

• Develop and implement personalized care plans in collaboration with clients, families, and healthcare professionals
• Monitor and report client progress to ensure optimal care and outcomes
