Clinical Nurse Leader Fellow (RN) Surgical Intensive Care and Intermediate Surgical Care

Full-time, 40 hours p/week

 

To facilitate the transition of BSN prepared Registered Nurses to the Clinical Nurse Leader (CNL) role by working to improve the quality of patient care outcomes while mentored by CNLs and Lean Six Sigma Black Belts and attending a graduate CNL program. Ensures coordination of patient care through the development and use of practice standards. Serves as a clinical expert providing analytical support to nursing leadership to plan and predict patient care needs based on quality benchmarking, research, and evidenced based practice.

Responsibilities

  • Performs tasks listed below mentored by a certified CNL and Lean Six Sigma Black Belt.
  • Designs, coordinates, integrates, and evaluates nursing care for assigned units(s) to ensure optimal patient outcome. Performs or oversees the collection of holistic assessments.
  • Collaborates with unit leadership to create an environment that facilitates and improves patient care delivery.
  • Synthesizes data, information, and knowledge to evaluate and achieve optimal patient outcomes
  • Provides leadership in the educational needs of patients and families and develops the clinical expertise of staff. Provides formal and informal educational opportunities on the unit to improve clinical skills of all staff.
  • Recommends specific programming to build clinical skill set of staff. Provides input and feedback to Manager related to evaluation and competency of staff.
  • Patriciates in staff meetings, huddles, and other department meetings.
  • Complete graduate CL program up to and including certification.
  • Performs other duties as assigned.
  • Qualifications

    • An eligible candidate must have at least three years nursing experience.
    • The candidate must have a written recommendation from their current manager.
    • A Bachelor’s degree in nursing is required.
    • The candidate must be accepted into a qualified CNL graduate program before their fellowship start date.
    • Preferred experience includes, but may not be limited to:
    • Performance improvement/Quality improvement committees and or projects
    • Participation in shared governance/local practice council
    • Safety champion participation
    • Experience with implementing unit initiatives
    • Preceptor experience
    • Leadership experience (supervisor, charge RN)
    • Preferred education/certifications include, but are not limited to:
    • Yellow or Green Belt training/certification
    • Specialty certifications
    • Quality or safety certifications

    Nurse Manager -Family Medicine Newport Health Center

    Overview At the direction of the Director of Clinical Services, and in collaboration with Ambulatory Leadership, provides guidance and leadership for the clinical practice of the staff in the ambulatory settings at Newport Health Center (NHC). Accountable for the quality of nursing care service, in accordance with the NH Nurse Practice Act, hospital policies, and the NH Board of Nursing rules for delegated nursing tasks. Collaborates with Medical Director, providers and Practice Leadership to assure development and ongoing competence of the clinical support staff. Works closely with the NLH Primary Care Operations team to build consistent clinical processes across locations. Collaborates with the CNO to assure that system nursing practice goals and expectations are met. Responsibilities Clinical Operations Directly responsible for operational and performance management of RN/LPN, CMA, and other clinical staff in the NLH Medical Group Practice (direct reports). Manages Clinical Schedule, Time Away, and Payroll for Direct Reports, and collaborates with Practice Leadership to provide consistent appropriate clinical coverage to NLMG Providers. Develops and implements program for the onboarding of new clinical staff. Provides leadership in emergent situations and in those that require activation of the Emergency Activation Plan. Maintains up-to-date working knowledge of emergency preparedness in order to function as a resource to staff. Maintains clinical skills in order to provide direct care as needed. Provides clinical consultation and assistance upon request to other patient care areas at NLH and NHC Develops, in collaboration with the Practice Leadership, CNO and nurse leaders, programs and strategies to assure ongoing competency of RN clinical staff. Maintains professional competence through participation in education programs and workshops. Collaborates with Clinical Educator in all clinical education opportunities and mandates. Quality Accountable for working with staff to assure Rural Health Center (RHC) survey readiness. Assists and supports Practice Leadership in the implementation and evaluation of performance improvement indicators. Functions as a "champion" in designated initiatives by role modeling, serving as an educator, and participating in the evaluation of the initiative. Serves as a resource to clinical staff for specialty-based information and research. Promotes clinical practice based on best evidence. Promotes a culture of patient-centered care.     Charge Nurse - Emergency Department

    Overview The ED Lead/Triage Nurse is a vital leadership and clinical role responsible for overseeing shift operations in the Emergency Department, ensuring efficient patient flow, safe care delivery, and effective team coordination. This role also involves assessing and prioritizing patients upon arrival through systematic triage, initiating appropriate care pathways, and facilitating communication between healthcare teams. Responsibilities Lead Nurse Responsibilities: Leadership & Oversight Serve as the clinical leader and primary resource for nursing staff during assigned shifts. Coordinate staffing assignments based on patient acuity and unit needs. Manage shift operations, including delegation of tasks and monitoring workflows. Support staff in escalating clinical concerns to providers or leadership as needed. Patient Flow & Throughput Oversee the movement of patients through the ED, from arrival to disposition (admission, transfer, or discharge). Collaborate with bed management, inpatient units, and ancillary departments to optimize patient flow. Address and mitigate bottlenecks or delays in real-time. Resource Management Ensure appropriate availability of supplies, equipment, and staffing resources. Initiate and lead code responses, disaster preparedness actions, and emergency protocols. Communication & Team Support Act as a liaison between ED staff, physicians, other departments, and hospital leadership. Facilitate shift handoff and ensure continuity of care through effective communication. Provide coaching, mentorship, and conflict resolution as needed. Triage Nurse Responsibilities: Initial Patient Assessment Perform comprehensive triage assessments using standardized triage protocols (e.g., ESI—Emergency Severity Index). Clinical Decision-Making Identify high-risk patients and initiate life-saving interventions per standing orders or protocols. Communicate critical findings promptly to the care team. Documentation & Data Entry Accurately document triage assessments, vital signs, chief complaints, and other pertinent clinical information in the EHR. Patient Experience & Safety Provide clear instructions and explanations to patients and families regarding wait times, processes, and care expectations. Ensure infection control and safety measures are adhered to in the waiting area and triage spaces. Qualifications Education: Graduate of an accredited Nursing Program (ADN or BSN preferred). Licensure: Current Registered Nurse (RN) license. Certifications: BLS and ACLS upon hire. PALS and AVADE within 6 months of hire. TNCC and NRP within 1 year of hire. ENPC is supported by leadership and optional. Experience: Minimum of 2–3 years Emergency Department nursing experience. Prior leadership or triage experience preferred. Skills: Strong clinical assessment and critical thinking skills. Excellent communication, leadership, and team coordination abilities. Proficiency in electronic medical records (EMR/EHR).     Overview Integrate emergency care for individual patients and implement nursing standards in the processes of care. Responsibilities Utilizes the nursing process in caring for all patients, to include: assessing symptoms, planning interventions, implementing treatment plans, evaluating responses to interventions, and revising plan based on overall effectiveness.
