Family Support Care LLC, will be implementing the KumZ Dementia Caregiver Relief Program, Butler County Pilot at the Middletown YMCA. This is a yearlong program from 7/01/2026-06/30/2027. Funding is supported by the Administration for Community Living (ACL) and U.S department of Health and Human Services (HHS). The Home Health Aide will offer people living with dementia safe supervision, and meaningful engagement. The goal of the project is to improve dementia specific respite care through a community based person-centered, predictable and measurable model.

The Home Health Aide will support people living with dementia in movement sessions that occur twice weekly lasting 90-120 minutes. Participate in team meetings, debriefings, set up and clean up. The Home Health Aide will report to the Lead staff team member for the Respite Program and complete all compliance grant related activities. The position is time limited and will end on or before 6/30/2027.

Deliver high quality, person-centered support to individuals with Alzheimer’s and related dementia

Bathroom Support: assist clients with bathroom support by assisting them as they use the restroom.

Behavioral and Emotional Support: Implement individualized support plans to address behavioral needs and manage challenging situations using de-escalation techniques and positive reinforcement.

Activity Facilitation: Plan and lead recreational, educational, and social activities to foster skill development, community integration, and personal growth (e.g. outings, hobbies, or vocational training).

Documentation and Reporting: Maintain detailed records of client progress, incidents, and daily activities; communicate effectively with supervisors, healthcare professionals, and family members.

Crisis Intervention: Respond to emergencies, provide first aid/CPR as needed and follow safety protocols to ensure client well-being.

Team Collaboration: Work collaboratively with teams, including therapists, SSAs, and other support staff, to develop and update individualized service plans (ISP).
