Overview

Heritage Home Healthcare is seeking a compassionate and dependable PRN Caregiver to provide personalized in-home care for a client in the San Ysidro, NM (87053) area. This position offers approximately 15 hours per week with a flexible schedule based on the client's needs.

In this role, you'll assist with daily living activities while promoting the client's comfort, safety, independence, and dignity. We're looking for someone who is reliable, observant, and passionate about providing high-quality, person-centered care.

Key Responsibilities
  • Assist with personal care tasks including bathing, dressing, grooming, toileting, and eating.
  • Follow the client's individualized care plan, including assisting with exercises and delegated medication assistance, as appropriate.
  • Provide mobility assistance, including transfers to and from beds, chairs, or wheelchairs.
  • Perform light housekeeping to maintain a clean and safe home environment.
  • Provide companionship, emotional support, and encouragement.
  • Escort or transport the client to appointments or errands, if needed.
  • Observe and report changes in the client's condition, behavior, or needs to the clinical team.
  • Follow all care plans, policies, and delegation instructions provided by licensed healthcare professionals.

Qualifications
