Caregiver - Evening and Day Shifts Available

We are seeking compassionate and dedicated In-Home Caregivers to provide high-quality care to our patients in Lakeville, MN. As a Caregiver with SYNERGY HomeCare, you will play a vital role in helping patients maintain their independence, dignity, and overall well-being in the comfort of their own homes.

Responsibilities:

Provide personal care services, including bathing, dressing, grooming, and toileting
Assist with meal preparation, medication management, and other daily living activities
Support patients with physical and cognitive disabilities, including Alzheimer's and dementia
Build strong relationships with patients and families, promoting emotional and psychological well-being
Collaborate with healthcare teams to develop and implement individualized care plans
Maintain accurate and detailed records of patient care and services provided
