What You’ll Do

 

Deliver Compassionate Hospice Care

  • Provide direct patient care in accordance with the Nurse Practice Act, with a focus on end-of-life support
  • Implement individualized plans of care under the supervision of a Registered Nurse
  • Deliver effective pain and symptom management, monitoring and adjusting based on patient response
  • Support patients with daily care needs while ensuring comfort, dignity, and safety
  • Educate patients and families on disease progression, medications, and comfort measures

Coordinate & Communicate

  • Document care accurately and timely in the EMR
  • Communicate changes in patient condition to the RN and care team
  • Collaborate with interdisciplinary partners (RN, HHA, Social Work, Chaplain, Therapy)
  • Participate in care planning and coordination to ensure holistic support

Commit to Excellence

  • Follow best practices and clinical standards in hospice care
  • Maintain proper use of equipment and adhere to infection control protocols
  • Participate in quality improvement initiatives and team development
  • Continue professional growth through training and education

 

What You Bring

  • Active LPN/LVN license in the state of practice
  • Current CPR certification
  • 1–3+ years of nursing experience preferred (hospice, home health, or med-surg)
  • Strong clinical judgment, communication skills, and emotional intelligence
