This a full timein-person position based out of Brooklyn, NY.

IMPORTANT– PLEASE READ BEFORE APPLYING:

This position requires hands-on advanced, administrative HHAeXchange experience in a CHHA/home care agency setting.

This is NOT a Home Health Aide position.

The ideal candidate must have hands-on experience using the administrative side of HHAeXchange, including broadcasting cases, reviewing agency responses, managing cases and authorizations, verifying Plans of Care, monitoring case acceptance/declines, and working within HHAeXchange to support CHHA operations.

Pay: $23.00 per hour-$26.00 per hour

Job Summary: The Home Care/ HHA Coordinator is responsible for managing CHHA cases and related administrative activities within the HHAeXchange platform, including case broadcasting, agency responses, authorizations, and Plans of Care. This position works closely with home care agencies and internal teams to ensure accurate case management, timely placement, and compliance with applicable requirements.

Job Responsibilities:

HHAeXchange System Management

  • Maintain accurate caregiver, patient, and authorization records in HHAeXchange.

  • Monitor and resolve scheduling, visit verification, and billing exceptions.

  • Process caregiver onboarding and profile updates within the system.

  • Ensure authorizations, service codes, and payer information are entered correctly.

Electronic Visit Verification (EVV)

  • Monitor EVV compliance and address missing or incomplete visit records.

  • Investigate and resolve visit exceptions, missed punches, and documentation discrepancies.

  • Work with caregivers and coordinators to correct attendance and visit verification issues.

  • Generate and review EVV compliance reports.

Scheduling Support

  • Assist with caregiver scheduling and shift coverage as needed.

  • Verify service hours against authorizations and approved care plans.

  • Coordinate schedule adjustments and communicate changes to stakeholders.

Compliance & Quality Assurance

  • Ensure documentation meets payer, Medicaid, and agency requirements.

  • Conduct routine audits of caregiver and patient records.

  • Track expiring credentials, licenses, and mandatory training requirements.

  • Support internal and external audits.

Reporting & Communication

  • Prepare operational, compliance, and performance reports.

  • Serve as a point of contact for caregivers regarding HHAeXchange-related questions.

  • Collaborate with Intake, Billing, Payroll, Clinical, and Operations teams to resolve issues.

  • Communicate effectively with managed care organizations (MCOs) and payer representatives.

Qualifications:

  • High school diploma required; Associate's or Bachelor's degree preferred.

  • 1-3 years of home care, healthcare operations, scheduling, or EVV experience.

  • Experience working with HHAeXchange.

  • Strong knowledge of Medicaid and home care compliance requirements.

  • Proficiency in Microsoft Office Suite, particularly Excel.

  • Excellent organizational, communication, and problem-solving skills.

  • Ability to manage multiple priorities in a fast-paced environment.
