RN Case Manager, Home Care

Own the clinical function from day one

Right at Home North Richmond is a licensed home care agency serving Henrico County. We are hiring our first Registered Nurse, and this is not a staff nursing job. You will own the clinical function: every assessment, every plan of care, every supervisory visit, and every clinical standard this agency operates under.

You report directly to ownership. There is no layer between you and the decision.

You are also not starting from a blank page. Right at Home has been in home care since 1995 and operates more than 750 locations across five countries, so the policy framework, training curriculum, and clinical documentation tools are already built and supported. Our Virginia home care license is issued and in hand. Your job is to run the clinical function well, not to invent it from scratch.

What Registered Nurse will actually do

  • Conduct initial in-home client assessments and write the plan of care

  • Perform supervisory visits and caregiver competency evaluations in the home

  • Coach and train caregivers on client-specific needs and safe technique

  • Take clinical escalations from caregivers and decide the next step

  • Hold the line on scope of practice with families, clearly and kindly

  • Own clinical documentation and keep records survey-ready under Virginia OLC regulations

Schedule and pay

  • $40 to $46 per hour, based on experience

  • Daytime hours that you help set. This role suits an RN who wants clinical ownership without a 12-hour floor shift

  • Paid travel time between clients, plus mileage reimbursed

  • W-2 employee, not 1099

Territory

You will travel across our service area: Glen Allen, Short Pump, Innsbrook, Lakeside, and Tuckahoe, out to Varina and Highland Springs. Our office is in Richmond's West End.

What Registered Nurse needs

  • Active Virginia RN license, or a multistate compact license carrying Virginia privilege

  • Two or more years of RN experience. Home health, hospice, or home care preferred

  • Current CPR/BLS certification

  • Valid driver's license and a reliable, insured vehicle available during the workday

  • Comfortable working independently in clients' homes

Outpatient Registered Nurse - RN - Dialysis

PURPOSE AND SCOPE: The professional registered nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. This may include delegation of appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs and Patient Care Technicians. SUPERVISION: Assigned oversight of Patient Care Technicians/LPNs/LVNs/RNs as a Team Leader or designated Nurse in Charge, after meeting all the following: · Successful completion of all FKC education and training requirements for new employees. · Must have a minimum of 9 months experience as a RN. · Must have a minimum of 3 months experience in chronic/acute hemodialysis as a RN. EDUCATION and LICENSURE: · Graduate of an accredited School of Nursing. · Current appropriate state licensure. · Current or successful completion of CPR BLS Certification. · Must meet the practice requirements in the state in which he or she is employed. EXPERIENCE AND REQUIRED SKILLS: · Entry level for RNs with less than 2 years of Nephrology Nursing experience as a Registered Nurse. · Chronic/acute hemodialysis experience (preferred).
