Position Summary

Delta Homecare is seeking compassionate, dependable, and caring individuals to join our team as Caregivers/Home Care Aides. The caregiver provides quality, person-centered care to clients in the comfort of their homes while promoting dignity, independence, safety, and overall well-being.

Essential Duties & Responsibilities

● Assist clients with activities of daily living, including bathing, dressing, grooming, toileting, and personal hygiene.
● Assist with mobility, transfers, positioning, and ambulation as appropriate.
● Prepare meals and assist with feeding when needed.
● Provide light housekeeping related to the client’s care and living environment.
● Assist with laundry, linens, and maintaining a safe and clean environment.
● Provide companionship and emotional support.
● Assist clients with maintaining independence and participating in daily activities.
● Remind clients about medications according to the established care plan and applicable regulations.
● Observe and report changes in the client’s condition, behavior, or needs to the appropriate supervisor.
● Follow each client’s individualized care plan.
● Maintain client confidentiality and protect personal information.
● Complete required documentation accurately and on time.
● Follow all company policies, procedures, and applicable Medicaid and regulatory requirements.
● Communicate professionally with clients, families, supervisors, and other members of the care team.
● Maintain a safe, respectful, and professional environment for clients.

Qualifications

● Previous caregiving or home care experience preferred.
● Must demonstrate compassion, patience, dependability, and professionalism.
● Must be able to communicate effectively with clients, families, and staff.
● Must be able to follow written and verbal instructions.
● Must successfully complete required background checks.
● Must meet all applicable state, Medicaid, and agency requirements for the position.
● CPR/First Aid certification preferred or willingness to obtain as required.
● Reliable transportation may be required depending on the assigned client/case.

Physical Requirements

The caregiver must be able to safely perform duties required by the client’s care plan, which may include standing, walking, bending, reaching, lifting, transferring, and assisting clients with mobility.

Provides dementia care including adapted ADL’s, meaningful engagement and communication to residents following a person centered individual service plan. Treats residents with respect and dignity, recognizes individual needs, and encourages independence. Fosters a purposeful and engaging culture throughout the community. Assists residents and provides input in regards to the care plan and daily living activities. This includes bathing, dressing, grooming, toileting, transferring and getting to and from programs and meals according to the individual service plan and individualized interventions. Partners and encourages residents to be as independent as possible encouraging them to do as much of their own care as possible. Participates in team behavioral expression meetings or discussions with community leadership. Knows the residents Life Story, preferences, historical routines and interests. Serves meals to residents in the dining room. May assist in preparing meals following preplanned dementia friendly menus. Encourages residents with prompting and cueing skills to eat a proper diet and take adequate fluids while respecting the resident's right to free choice. Records and reports changes in resident's eating habits to the supervisor. Initiates and/or leads minimally one Daily Path engagement program provided for residents as described in the resident’s Life Story, Solace program or calendar. Encourages residents to socialize and participate in planned, spontaneous, individual or small group person centered programs and to support friendships between residents. Maintains residents' records daily, in a timely manner, and in accordance with company policy and procedures.
