Responsibilities

  • Deliver direct patient care in accordance with care plans and physician orders

  • Supervise, guide, and support CNAs to ensure high-quality care

  • Administer medications and treatments as prescribed

  • Monitor residents’ conditions and report changes promptly

  • Coordinate resident admissions, discharges, and transfers

  • Maintain accurate documentation in PointClickCare (PCC)

  • Foster resident-centered care and build strong relationships with families

Qualifications

  • Active, unrestricted Illinois RN or LPN license

  • Current CPR certification

  • Strong communication and teamwork skills

  • Ability to provide compassionate, quality care in a fast-paced environment

LPN/RN will participate in the delivery of patient care in accordance with facility clinical systems, regulatory requirements and physician orders. Nurses oversee the direction of the Nursing Assistants to help and ensure that the highest quality services are delivered in a resident-centered environment. Possess and maintain a current and unrestricted IL LPN/RN License CPR certification Excellent verbal and non-verbal communication skills Skilled nursing facility experience preferred JOB DESCRIPTION: Be responsible for the care and wellbeing of all.
