Job Summary:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Night Shift team at Alden Long Grove Rehabilitation and Health Care Center. As a key member of our care team, you will provide high-quality, patient-centered care to our residents, ensuring their physical, emotional, and spiritual needs are met. Responsibilities:

  • Provide direct patient care, including administering medications, wound care, and treatments, under the supervision of a registered nurse or physician

  • Monitor and report patients' conditions, including changes in vital signs, behavior, and symptoms

  • Develop and implement individualized care plans in collaboration with the interdisciplinary team

  • Maintain accurate and detailed records of patient care and treatments

  • Participate in care conference meetings to discuss patients' progress and concerns

  • Collaborate with other healthcare professionals to ensure seamless care transitions and communication
