Responsibilities:

Conduct comprehensive patient assessments and reassessments.
Manage and track case progress, ensuring timely follow-ups and completing necessary assessments.
Maintain thorough documentation and stay on top of assessment dates.
Provide high-quality care and support to patients as needed.
Communicate effectively with patients, families, and the homecare team.

Requirements:

Valid RN license in New Jersey.
Reliable car and valid driver's license.
Willingness to travel throughout New Jersey.
Strong case management skills with the ability to manage multiple patients.
Excellent organizational skills and attention to detail.

Where: Visiting Angels Registered Nurse (RN) to visit client in their homes throughout Burlington County Shift Type: Per Diem Compensation: $70/visit for initial assessment and $60/visit for 60-day visits.
