Qualifications:

  • Must meet Medicare Conditions of Participation for Home Health Aide training program and competency. If currently a licensed CNA, GPHH can provide Home Health Certification

  • Have a sympathetic attitude toward the care of the sick and elderly.

  • Ability to carry out directions, read and write.

  • Maturity and ability to deal effectively with the demands of the job.

 

Principal Responsibilities:

This position reports to Home Health Director and is supervised by RN’s, PT’s, OT’s, SLP’s

  • Understands and adheres to established Agency policies and procedures.

  • Performs personal care, bath, and hands-on care as assigned.

  • Completes appropriate visit records in a timely manner as per Agency policy.

  • Reports changes in the patient's condition and needs to the RN.

  • Performs household services essential to health care in the home as assigned.

  • Ambulates and exercises the patient as assigned.

  • Performs simple procedures as an extension of the therapy or nursing services, e.g., range of motion (ROM) exercises as assigned.

  • Assists with medications that are ordinarily self-administered as assigned.

  • Attends in-service and continuing education programs as scheduled and necessary.

  • Attends patient care conferences as scheduled.

    Preferred Qualifications

    • Home care and home health background.

    • Experience working in a home healthcare environment.

    • Familiarity with healthcare regulations, HIPAA compliance, and patient privacy requirements.

    • Knowledge of the local healthcare market and community outreach strategies.

    • Knowledge of appropriate software including Microsoft Word, Excel, Outlook, Wellsky

    • Knowledge of office management systems and procedures.

    • Clean driver’s license with a clean driving record (Preferred).
