We are seeking a compassionate and dedicated Caregiver (Home Health Caregiver) to join our team at Right at Home Central MO. As a Caregiver, you will play a vital role in providing exceptional care and assistance to seniors and vulnerable adults in their homes.

About the Role:

Provide one-on-one care and companionship to clients in their homes, including activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
Assist with personal care services such as bathing, dressing, and grooming
Offer light housekeeping, meal preparation, and other household tasks
Engage with clients in recreational activities, conversation, and socialization

Responsibilities:

Conduct initial client assessments to understand their unique needs and preferences
Develop and implement customized care plans in collaboration with clients, families, and healthcare professionals
Provide ongoing care and support to clients, adjusting plans as needed
Maintain accurate records of client interactions, care provided, and any concerns
Communicate effectively with clients, families, and healthcare teams to ensure seamless care coordination

Work Environment:

Work as part of a dynamic and compassionate team
Provide care in clients' homes, with occasional travel to Right at Home Central MO offices
Work varying shifts, including days, evenings, and weekends
