Job Summary:

As a Nurse Practitioner with Carease Health you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. We're seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to join our team.

About the Role:

As a Nurse Practitioner with Carease Health you will:

  • Deliver comprehensive preventative care to residents in skilled nursing facilities

  • Conduct on-site visits and provide patient-centered care, supported by remote monitoring and chronic care management

  • Build strong relationships with facility staff and patients, maintaining a patient-centered approach

  • Collaborate with our clinical directors, regional managers, and supervisors to ensure high-quality care

  • Participate in quality improvement initiatives to minimize hospitalizations and elevate patient care standards

Requirements:

  • Active NP or PA license (in the state of practice)

  • 1+ year of experience in Geriatric care and documentation review
