We are currently hiring for the Scranton/Honesdale/Clarks Summit, Pennsylvania area, including surrounding communities.

Schedule

  • Full-Time

  • Monday–Friday

  • 8:30 AM – 4:30 PM

  • No nights or weekends!

Position Summary

As a Hospice RN Case Manager, you will coordinate and manage hospice care for patients in their homes, assisted living communities, and skilled nursing facilities throughout the Scranton/Honesdale territory. Working as part of an interdisciplinary team, you will develop individualized care plans that promote comfort, dignity, and quality of life while supporting patients and their loved ones throughout their hospice journey.

Responsibilities

  • Complete comprehensive hospice admission assessments for patients and families.

  • Develop, implement, and regularly update individualized hospice care plans.

  • Perform ongoing nursing assessments and monitor changes in patient condition.

  • Coordinate care with physicians, hospice aides, LPNs, social workers, chaplains, and other members of the interdisciplinary team.

  • Educate and support patients and families regarding hospice services, symptom management, and end-of-life care.

  • Initiate appropriate nursing interventions to maximize patient comfort.

  • Communicate changes in patient condition to the attending physician and hospice medical director as appropriate.

  • Supervise Hospice Aides and Licensed Practical Nurses (LPNs).

  • Maintain accurate, timely, and compliant clinical documentation.

  • Participate in patient visits, case management, and team coverage as needed.

Qualifications

  • Current Pennsylvania Registered Nurse (RN) license in good standing.

  • Hospice nursing experience preferred; home health or palliative care experience is a plus.

  • Strong clinical assessment, communication, and organizational skills.

  • Reliable transportation, valid driver's license, and current automobile insurance.

  • Ability and willingness to travel throughout the Scranton/Honesdale service area.
